Name Change in LIC Policy in Jharkhand
An LIC policy is the one record whose name error your family discovers, not you, at the claim counter years later. It is also the one with no form: LIC publishes no name change form and no document list, the request is a letter to the servicing branch, and the change is endorsed on the original policy bond, which is why nothing about it is online. This page is the insurance step for a Jharkhand resident: the letter and what it encloses, the proof by reason, the bond and the servicing branch, the two Jharkhand divisions under LIC's East Central Zone, the nominee's name as a separate request on Form 3750, private insurers and health policies, the Central Gazette process before a chosen change, what a wrong name does to a claim, and the Bima Bharosa and Ombudsman ladder when a branch stalls.
A name change in LIC policy in Jharkhand is a written request to the branch manager of the servicing branch, the branch printed on your premium receipt, listing every policy number, the name on record and the exact new name, with the original policy bond for endorsement, the proof for your reason, the registered marriage certificate for a surname after marriage, the decree for a maiden name, or the Gazette of India for any other change, and an identity document in the new name, usually the updated Aadhaar, with the old one where asked. LIC publishes no name change form and no document list; the letter is the form and the endorsement on the bond is the change, which is why it cannot be done through e-services. The Citizens' Charter allows seven days for a correction. A nominee's name is a different request: replacing a nominee is Form 3750, and correcting a nominee's spelling is a letter with the nominee's ID. A Jharkhand policy sits under the Hazaribagh or Jamshedpur division of LIC's East Central Zone at Patna, and the escalation runs branch, division, zone, Bima Bharosa, Ombudsman.
The pages ranking for name change in LIC policy in Jharkhand invent what LIC does not publish: "attested name change documents by a gazetted officer", "a copy of the newspaper advertisement", "a quotation fee", and a "nominee change form" that is somehow also the name change form. One in our own network says a "Policy Servicing Form" is available at the branch, which is not LIC's name for anything. The accurate account is shorter and stranger: there is no form, the letter is the form, the bond is the document, and the branch that holds the policy is the only office that can act.
This page keeps LIC in its place, at stage six of the Jharkhand record order with EPFO and NPS, after the banks, and separates the three things people bundle together: the policyholder's name, the nominee's name, and the bank account the claim will be paid to. The banks are on the bank records page; the Aadhaar that serves as the new-name identity document is on the Aadhaar page.
At a glance
- The request
- A signed letter to the Branch Manager of the servicing branch; no LIC form exists for a policyholder's name change
- The document
- The original policy bond, endorsed and returned; without it the branch cannot complete the job
- Proof by reason
- Marriage certificate; decree; or the Gazette of India; with Aadhaar in the new name and the old one where asked
- Not needed
- A gazetted officer's attestation, a newspaper cutting, an affidavit for a marriage change; LIC lists none of them
- Fee
- None published for a name correction; the branch states any service charge in writing
- Time
- Seven days for a correction under the Citizens' Charter; the endorsed bond by hand or post
- Online
- No. e-services show the bond image, status and premiums; a name change is endorsed physically
- The nominee
- A separate request: Form 3750 to replace a nominee; a letter with the nominee's ID to correct a spelling
- Jharkhand
- Hazaribagh Division (Jeevan Prakash, Julu Park, Hazaribagh 825301) and Jamshedpur Division (Jeevan Prakash, Main Road, Bistupur, Jamshedpur 831001), East Central Zone, Patna
- Escalation
- Branch, division CRM, zonal office, Bima Bharosa, Insurance Ombudsman at Patna
Key takeaways
- There is no LIC name change form. Write the letter well, because the letter is the application.
- The bond has to go to the branch; the change is an endorsement on it. Nothing here is online and no page that says otherwise is describing LIC.
- Marriage needs the certificate; divorce the decree; a chosen change the Gazette of India. A gazetted officer's attestation and a newspaper cutting are inventions.
- Your name, the nominee's name and the claim account are three requests. Decide at home which you need and make one trip.
- A policy is serviced by the branch that holds it, wherever you live now; a Ranchi branch cannot endorse a Dhanbad policy without a transfer.
- Fix it while you are alive. A name mismatch found at a death claim is your family's problem, with an affidavit and an indemnity, not yours.
The Letter for a Name Change in LIC Policy: Because There Is No Form
LIC's e-services list what a policyholder can do online: policy status, bonus, loan and claim status, premium certificates, an image of the bond. A name change is not among them, and LIC publishes neither a form nor a document list for it. The request is therefore a letter to the Branch Manager of the servicing branch, and its quality decides whether the endorsement is done in one visit.
What the letter says
- To the Branch Manager, LIC of India, [servicing branch and code].
- Subject: change of name of the policyholder in policy numbers [list every one].
- The name on record: [old name, exactly as printed on the bond].
- The new name: [exactly as on Aadhaar], and the reason: marriage on [date] / Gazette of India notification [number, date, page].
- A request that the policy records and the bond be endorsed, and that a fresh premium receipt in the new name be issued.
- Enclosures listed; old signature; new signature if changed; date; mobile and email on record.
What it encloses
- The original policy bond, for endorsement and return.
- The proof for the reason, original shown, self-attested copy kept.
- Aadhaar in the new name, and where the branch asks, the identity document in the old name that matches the bond.
- PAN, because the claim and the maturity are paid against it.
- A photograph, and the latest premium receipt.
A signature change goes on the same visit on the branch's signature form; a bank account change for the claim, on the NEFT mandate form with a cancelled cheque in the new name.
Documents for a Name Change in LIC Policy in Jharkhand, by Reason
LIC publishes no list, so the branch confirms; this is what the branches accept in practice, and what they do not need.
| Reason | Proof | Gazette? |
|---|---|---|
| Surname after marriage | Registered marriage certificate; Aadhaar in the new name | No |
| Maiden name after divorce | Certified decree; Aadhaar in the new name | No |
| Spelling error on the bond | Aadhaar, PAN or the Class 10 certificate showing the correct form; the proposal form's spelling if the error was LIC's | No |
| Full or first name change by choice | The Gazette of India notification, printed; Aadhaar in the new name | Yes, the Gazette of India; Jharkhand has no State Gazette route |
| Wrongly written name corrected by public notice | The public notice PDF; Aadhaar | The public notice serves as the Gazette |
| Minor's policy, name changed by the parent | The Gazette of India minor notice or the adoption order; the proposer's ID | Yes, for a chosen change |
Central Gazette Name Change Process From Jharkhand, Step by Step, Before the LIC Policy Endorsement
For a chosen new name, LIC and every insurer want the Gazette of India as the instrument, and the Gazette of India notification. There is no Gazette office in Jharkhand and no State Gazette route; the file goes to the Department of Publication in Delhi in your own name. Here is the whole Central Gazette process, in the order it happens.
Route check and wording, day 0
Confirm that you actually need a gazette: a marriage surname, a divorce or a spelling correction do not. Then fix the exact new name, spacing and initials included, against your Class 10 certificate, and confirm it in writing. Every later paper copies this wording.
Affidavit, day 1 to 2
Sworn on non-judicial stamp paper before a notary at your district court, in the OLD name as on your current Aadhaar: old name, new name, father's or husband's name, address with pin code, reason, verification clause, notary seal and register number. Stamp paper and notary commonly Rs 100 to Rs 500. Format on the affidavit page.
Two newspaper notices, day 3 to 7
One Hindi daily and one English daily in editions that circulate in Jharkhand, identical twenty-word wording quoting the affidavit number and date. Keep the complete original page of each paper, masthead and date visible, two copies. Rs 800 to Rs 4,000 at actual cost. Detail on the newspaper page.
Proforma, CD and request letter, day 7 to 8
The Department's proforma typed in two copies, headed "No. ADVT IV /", signed in the old name, two witnesses with full addresses and pin codes, two photographs, self-attested IDs of you and both witnesses; a CD with the notice in Word without the witness portion plus a signed certificate that soft and hard copy match; a request letter to the Controller of Publications. Full list on the documents page.
Fee on BharatKosh, day 8
Only on bharatkosh.gov.in, in your own name, selecting the Department of Publication and the Gazette notification purpose: commonly Rs 1,100 for an adult, Rs 1,700 for a minor, revised each financial year, non-refundable. Print the receipt for the file. No demand draft, no cash, no counter anywhere in Jharkhand.
Delivered in Delhi in your name, day 8 to 10
The five papers, affidavit and pages in original, to the Department of Publication, Civil Lines, behind Delhi Vidhan Sabha Metro Station, Delhi 110054, in person or by tracked post in the applicant's name. Our Delhi office delivers Jharkhand files in your name. Nothing is returned, so keep a scanned set.
Published and downloaded, day 40 to 55
Part IV is released weekly; publication commonly takes 30 to 45 days. Search your old or new name on egazette.gov.in, download the free PDF, and note the Gazette number, date and page. No attestation is needed for use in India. A wrong print is corrected by corrigendum within six months.
Then the policy endorsement
Carry the printed Gazette PDF with the affidavit and both newspaper pages to Aadhaar and PAN first, then to the servicing branch with the bond, as the sections below explain. The address in the Gazette should match your Aadhaar address. The gazette pillar, with the July 2025 story of the Jharkhand Gazette, is the gazette name change page.
The LIC Name Change at the Servicing Branch, Step by Step
Aadhaar and PAN first, bond in hand
The identity document in the new name is what the endorsement is made against, and the maturity or claim is paid against PAN. Find the bond; a lost bond means a duplicate policy first, which is its own indemnity and fee.
Find the servicing branch
It is printed on the premium receipt and shown in e-services. A policy bought in Dhanbad and serviced there is not endorsed at Ranchi; either go to the servicing branch or ask for a policy transfer to the branch near you, a separate request that takes weeks.
Submit and take the acknowledgement
The letter with every policy listed, the bond, the proof, Aadhaar, PAN, photograph, premium receipt; the nominee form and the NEFT mandate on the same visit if those change. The branch verifies originals, stamps the copy, and keeps the bond for endorsement.
The endorsed bond and the check
Within the Charter's seven days for a correction, the bond comes back with the endorsement slip or the change noted on it. Check the next premium receipt and the e-services policy details for the new name; keep the endorsed bond with the Gazette page.
The full record order is on the name change process page; LIC sits at stage six with EPFO and NPS.
The Nominee's Name on an LIC Policy: Form 3750 and the Correction Letter
A nomination under Section 39 of the Insurance Act is a record on the policy of who receives the claim, and LIC changes it only by an entry endorsed on the bond, which is why nominee changes are also not online. Two different things get called "nominee name change". Replacing the nominee, a wife instead of a mother after marriage, a child added, is Form 3750, with the new nominee's ID, proof of relationship and the bond; an appointee is named where the nominee is a minor. Correcting the spelling of an existing nominee's name, or recording a nominee's own name change after her marriage, is a letter with the nominee's Aadhaar and, for a change, her marriage certificate; the nominee does not have to attend. The first change of nomination is free on smaller policies and carries a small fee on others, stated by the branch.
Replace the nominee
Form 3750, the bond, the nominee's ID, relationship proof; appointee for a minor. Endorsed on the bond. Any number of times during the term.
Correct the nominee's name
A letter with the nominee's ID showing the correct form; her marriage certificate if her surname changed. No Form 3750, because the person is the same.
Why it matters more than yours
The claim is paid to the nominee as named. A nominee named "Sunita Kumari" who is now "Sunita Verma" on Aadhaar proves identity at the claim with an affidavit and delay; a corrected nomination avoids both. The marriage page lists the nomination in its sequence.
LIC in Jharkhand: The Two Divisions and the Servicing Branch
LIC serves Jharkhand from its East Central Zone, headquartered at Jeevan Deep, Exhibition Road, Patna, through two divisional offices inside the state and, for the Santhal Pargana, the Bhagalpur division across the border. A policy's servicing branch belongs to one of these divisions, and the division's customer relations manager is the first escalation above the branch.
| Division | Office | Branch areas, indicative |
|---|---|---|
| Hazaribagh Division | Jeevan Prakash, Julu Park, Hazaribagh 825301 | Hazaribagh, Giridih, Dhanbad, Katras, Chirkunda, Govindpur, Sindri, Bokaro, Bermo, Gomia, Ramgarh, Koderma, Chatra |
| Jamshedpur Division | Jeevan Prakash, Main Road, Bistupur, Jamshedpur 831001 | Jamshedpur, Adityapur, Jugsalai, Mango, Ghatsila, Chaibasa, Chakradharpur, Ranchi, Hinoo, Khalari, Gumla, Lohardaga, Simdega, Khunti, Medininagar, Garhwa, Latehar |
| Bhagalpur Division (Bihar) | Bhagalpur, Bihar | Dumka, Deoghar, Godda, Sahibganj, Pakur, Jamtara, as historically mapped; confirm on the LIC branch locator |
Private Life Insurers, Health and General Policies, and PMJJBY in Jharkhand
Private life insurers
HDFC Life, ICICI Prudential, SBI Life, Max and the rest take the same proof by reason, usually through a service request on the portal or app with uploads and an OTP, and endorse the policy schedule digitally. Nominee changes are often online too. The document set is the same; the trip is not.
Health and general insurance
A health policy, a motor policy or a home policy is endorsed by the insurer on the same proof; the motor policy follows the RC, which is on the driving licence page. A cashless claim is checked against the insured's name on the card and Aadhaar, so the health policy is done with the banks, not left for later.
PMJJBY, PMSBY and group covers
The Jan Dhan-linked life and accident covers follow the bank account's name and Aadhaar; they update when the bank does. Employer group covers at CCL, SAIL, Tata Steel and the State follow the employee record, which is the departmental deed route on the gazette page.
LIC Name Change Fees and Timelines in Jharkhand
| Item | Amount or time | Note |
|---|---|---|
| Policyholder's name correction or change | No fee published by LIC | The branch states any service charge in writing; a "quotation fee" is a duplicate-policy or alteration term, not a name one. |
| Correction under the Citizens' Charter | 7 days | From a complete submission at the servicing branch. |
| Change of nomination, Form 3750 | First change free on smaller sums assured; a small fee on others | Stated by the branch; endorsed on the bond. |
| Duplicate policy, if the bond is lost | Indemnity bond, advertisement where LIC asks, and LIC's fee | Weeks; do this before the name change. |
| Policy transfer to a nearer branch | Free | Weeks; a separate letter. |
| The proof before it: marriage certificate or decree | Registrar's or court's copying fee | No gazette. |
| The proof before it: Gazette of India | Commonly Rs 1,100 adult, Rs 1,700 minor, on BharatKosh | Revised each financial year, non-refundable. Full bill on the cost page. |
| Our charge | Usually nothing for the LIC step; the route check is free | Quoted per case where the Gazette is needed. |
What a Wrong Name on an LIC Policy Does at a Maturity or Death Claim
At maturity, while you are alive
- The maturity is paid by NEFT to the bank account on record, in the name on record, against PAN. A bond in the old name and an account in the new one is a mismatch the branch resolves with your affidavit and Aadhaar, and a delay.
- A policy loan or surrender is checked the same way.
At a death claim, when you cannot fix it
- The claimant proves that the deceased on the death certificate is the life assured on the bond. A mismatch means a one-and-the-same-person affidavit from the claimant, an indemnity, sometimes a succession angle, and months.
- A nominee named in a name she no longer uses proves her own identity the same way.
- Every line of this is avoided by a letter and one branch visit now.
LIC Branch Not Endorsing the Name Change: The Escalation
Branch, in writing
Ask the branch's grievance redressal officer, named on the notice board, to note the ground of refusal on your letter. Most stalls are a missing bond or a branch that is not the servicing one.
Division and zone
The customer relations manager at the Hazaribagh or Jamshedpur divisional office, then the East Central Zonal office at Patna, with the acknowledgement and the letter. LIC's grievance portal takes the same complaint online.
Bima Bharosa
IRDAI's integrated grievance portal, which registers the complaint with the insurer and tracks the 15-day response; the toll-free line does the same.
The Insurance Ombudsman
After 30 days without resolution, a free complaint to the Insurance Ombudsman whose centre covers Jharkhand, at Patna, under the Insurance Ombudsman Rules, 2017; deficiency in service on a policy correction is within scope.
LIC Name Change Mistakes That Cost a Second Trip to the Branch
Before the visit
- Going without the bond, or to a branch that is not the servicing one.
- Trying e-services or a call centre for a change that is a physical endorsement.
- Listing one policy when you hold four.
- Going before Aadhaar carries the new name, so there is no new-name identity document to endorse against.
Bundling and documents
- Filing Form 3750 to correct a nominee's spelling, or a letter to replace a nominee.
- Carrying a gazetted officer's attestation and a newspaper cutting because a blog listed them, and being told they are not needed.
- Changing your name and leaving the claim account, the nominee and the signature in the old one.
- Not checking the next premium receipt for the new name.
LIC Name Change Terms Explained for Jharkhand
- Servicing branch
- The LIC branch that holds the policy record, printed on the premium receipt and shown in e-services. The only office that can endorse a change, wherever the holder lives.

- The original policy document. A name change is endorsed on it, so it must go to the branch; a lost bond needs a duplicate policy first.
- Endorsement
- LIC's entry on the bond recording a change: the policyholder's name, the nominee, an address. The change is not effective until endorsed.
- Citizens' Charter
- LIC's published service standards, which allow seven days for a correction in policy records.
- Form 3750
- LIC's change of nomination form, for replacing a nominee; not a name correction form.
- Section 39 nomination
- The nomination under the Insurance Act, 1938 naming who receives the claim; registered only by endorsement on the bond.
- Appointee
- The adult named to receive the claim on behalf of a minor nominee.
- East Central Zone
- LIC's zone at Jeevan Deep, Exhibition Road, Patna, covering Bihar, Jharkhand and Odisha through its divisions.
- Hazaribagh and Jamshedpur divisions
- LIC's two divisional offices inside Jharkhand, each with a customer relations manager who hears escalations from its branches.
- Bima Bharosa
- IRDAI's integrated grievance management portal, the step between the insurer and the Ombudsman.
- Insurance Ombudsman
- The free forum under the Insurance Ombudsman Rules, 2017 for complaints unresolved after 30 days; the Patna centre covers Jharkhand.
- NEFT mandate
- The bank account form on which LIC pays maturities, survival benefits and claims; updated with a cancelled cheque in the new name on the same visit.
