The process then creates three more papers: the notarised affidavit, two complete newspaper pages (one Hindi, one English), and the Gazette proforma typed in two copies. Those go with a CD certificate and a request letter, as a five-paper file, to the Department of Publication, Civil Lines, Delhi 110054, with the BharatKosh receipt for the fee, commonly Rs 1,100 for an adult. A marriage certificate, divorce decree, child's birth certificate, custody order, departmental deed or the certificates carrying a wrong spelling are added only when the case calls for them. Witnesses, Photographs and Self-Attestation for a Jharkhand Name Change File
Two witnesses
Any two adults who know you: a relative, neighbour, colleague or friend. They sign the proforma in their own names, print their full names and full addresses with pin codes, and a self-attested copy of each witness's ID goes in the file. They do not appear before the notary and do not sign the affidavit. One witness, or a witness address without a pin code, returns the file.
Two photographs
Recent passport-size, plain background, one pasted on each copy of the proforma. For a minor, the child's photographs; the parent signs. Keep two spares for the Aadhaar Seva Kendra later.
Self-attestation
Every copy of an ID goes in signed by its owner across the copy with the date: yours across your Aadhaar copy, each witness across their own. Notary attestation is for the affidavit only; the copies are self-attested.
Sign in the old name, everywhere. The affidavit, the proforma and your self-attested copies all carry the name currently on your Aadhaar. The new name exists on nothing yet, and the file is the instrument that creates it.

Scan and Copy Rules for Name Change Documents in Jharkhand
Photographs of documents taken at an angle are the second most common reason a file waits a week.
How to send scans on day 0
- Flat, straight, all four corners visible, no glare, no finger over a name.
- Both sides of Aadhaar; the full page of a certificate including the number and date.
- One document per image; PDF or JPEG, readable at normal size.
- Type the new name in the message, not in a photo of handwriting.
Freshness and dates
- Documents in the file should not be older than about one year at filing.
- The affidavit and the newspaper pages should carry dates within the same few weeks.

- Keep a full scanned set of the finished file before it leaves; the Department returns nothing.

Document Mistakes That Return a Name Change File From Delhi
Missing or wrong papers
- One witness, or a witness address without a pin code.
- Newspaper cuttings, or two pages in the same language.
- Proforma in one copy, or handwritten, or signed in the new name.
- Soft copy as PDF instead of Word, or including the witness portion.
- No BharatKosh receipt, or a receipt in someone else's name.
- Copies not self-attested, or attested by the notary instead of the owner.
Mismatched papers
- Spelling differs by a letter between affidavit, newspaper and proforma.
- Address in the affidavit differs from the Aadhaar address.
- Father's name spelt one way on the affidavit and another on the Class 10 certificate.
- Affidavit a year old, pages last week.
- A wrong certificate spelling "corrected" in the affidavit instead of quoted exactly.
- Change-of-name papers paired with a public notice proforma, or the reverse.

Name Change Document Terms Explained for Jharkhand
- Self-attested copy
- A photocopy signed and dated by its owner across the face, in place of notary attestation. Every ID copy in the file is self-attested by the person it belongs to.
- Old-name ID
- A photo ID carrying the name you currently hold, usually Aadhaar. It is the name you sign in on every paper, because the new name exists on nothing yet.
- Witness
- An adult who knows you and signs the Gazette proforma with full name and full address including pin code. Two are required; their self-attested ID copies go in the file.
- Proforma
- The Department's change of name form, computer typed in two copies, headed "No. ADVT IV /", signed in the old name and witnessed. It carries the exact text that prints in Part IV.
- Minor specimen form
- The proforma variant headed "SPECIMEN FORM FOR CHANGE OF NAME OF MINOR", where the parent or guardian is the applicant and the child signs nothing.
- CD certificate
- A signed statement that the Word file on the CD matches the printed proforma, with the witness portion omitted and the old name typed in place of the signature.
- Request letter
- The covering letter to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054, with the BharatKosh receipt attached.
- BharatKosh receipt
- The printed receipt from the Non-Tax Receipt Portal proving the Government fee was paid, in the applicant's own name. The only accepted proof of payment.
- Complete page
- The full newspaper sheet carrying the notice, masthead and date intact. The Department accepts pages, not cuttings.
- Anchor document
- The record other records are judged against, for most Jharkhand residents the JAC Class 10 certificate. In a correction case its copy goes in the file.
