Hospital Discharge Summary and Medical Records Name Correction in Jharkhand
A discharge summary, the final bill, test reports and a medical certificate are the papers a hospital gives you. They are used for an insurance claim, a reimbursement from your office, medical leave, or a death claim later. Admission desks often type a name from memory, so a patient becomes "Sunita Devi" on the bill and "Sunita Kumari" on the policy. Only the hospital can correct its own papers, and it is easiest before you leave. We publish Gazette notices, and we say this first: a spelling slip in hospital records needs no Gazette notice.
Five points, in the order you will need them. First, check the patient's name on every paper before you sign the discharge, against the policy and Aadhaar. Second, if it is wrong, ask the hospital's billing or records desk for a corrected paper, signed and stamped, not a pen correction. This is our advice. Third, after discharge, write to the medical superintendent or records department, and ask for copies of the records; the 2002 medical ethics regulations say records should be issued within 72 hours of a request. Fourth, tell the insurer or office in writing that a correction is on its way. Fifth, a spelling slip does not need a Gazette. We publish Gazette notices, and we say this plainly.
What the Rules Say About Hospital Medical Records in India
We read the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002, and a news report from August 2026 on checking discharge papers. The correction advice further down is ours. These regulations bind doctors; each hospital has its own records desk and its own form. Check with the National Medical Commission whether newer regulations apply to your case.
| Point | What the sources show |
|---|---|
| Keeping records | Indoor patients' records must be kept for 3 years from the start of treatment, under Regulation 1.3.1 |
| Getting copies | A request by the patient, an authorised attendant or a legal authority should be acknowledged, and records issued within 72 hours, under Regulation 1.3.2 |
| Medical certificates | The doctor must keep a register of certificates, with the patient's identification mark and signature or thumbmark, under Regulation 1.3.3 |
| Before discharge | Check name, age, hospital number and dates against insurance records, and correct a wrong name before signing, not by hand later, as reported |
| Family policies | Make sure the papers name the correct insured member, as reported |
| Who corrects | The hospital, for its own papers. No outside office can |
So the hospital is the only place to fix a hospital paper, and the register entry is what matters. A corrected discharge summary that does not match the hospital's own register can raise questions later. This is our reading.
Which Hospital Record Name Mistake You Have
Name problems in hospital papers fall into a few kinds. This sorting is ours, based on the papers families send us.
| What does not match | What it usually is |
|---|---|
| Patient's name spelt wrong | The admission desk typed it from speech |
| Devi, Kumari or surname differs | The policy and the hospital used different forms |
| Husband's or father's name wrong | The "relation" field was filled carelessly |
| Age or gender wrong | Not a name slip, but it stops claims just the same |
| Wrong family member named | The policyholder's name used instead of the patient's |
| Bill and discharge summary differ | Two desks, two spellings |
| Policy itself is wrong | The hospital is right; the insurer must correct the policy |
How to Correct a Name on a Discharge Summary or Hospital Bill
Each hospital paper is corrected by the hospital. The table below is our practical advice.
| Situation | What we usually advise |
|---|---|
| Still in hospital | Show Aadhaar and the policy card at the billing desk. Ask for the record to be corrected before the final bill |
| Already discharged | Written application to the medical superintendent or records department, with Aadhaar, policy and the wrong papers |
| Bill and summary differ | Ask for both to be reissued in one spelling |
| Medical certificate wrong | Ask the doctor who signed it for a fresh one, entered in the register |
| Ayushman Bharat hospital stay | Also match the card. See Ayushman Bharat card name change |
| Patient's Aadhaar is the wrong one | Fix Aadhaar. See Aadhaar name change |
| Patient has since died | See death certificate and legal heir certificate |
Keep the application short: the patient's name as printed, the correct name, the hospital or IP number, the admission and discharge dates, and copies of Aadhaar and the policy. Ask for a stamped receiving copy, and a date by which the corrected papers will be ready. This is our advice.
If the hospital is slow, remind it that the 2002 regulations ask doctors to issue records within 72 hours of a request. Where a small gap remains, such as Devi and Kumari, a one-and-the-same affidavit may help the insurer accept the papers. The insurer decides, not us.
Insurance, Reimbursement and Other Uses of Hospital Records
Corrected hospital papers are usually needed for one of these. This list is our advice.
| Use | What to watch |
|---|---|
| Health insurance claim | See health and motor insurance |
| Central Government reimbursement | See CGHS card correction |
| Ex-servicemen | See ECHS card correction |
| ESIC benefits | See ESIC records |
| Digital health records | See ABHA health ID |
| Disability certificate | See disability certificate and UDID |
| Accident or death claim | See PMJJBY and PMSBY claim |
Does a Hospital Record Name Correction Need a Gazette Notice?
No, for a spelling slip, a Devi or Kumari difference, or a relative's name slip. Those are corrected by the hospital. A Gazette notice is for a genuine change of name, and even then the hospital still corrects its own papers.
| Situation | What usually applies |
|---|---|
| Discharge summary slip | Hospital correction. No Gazette |
| Bill or certificate slip | Hospital correction. No Gazette |
| Name after marriage | Marriage certificate. No Gazette |
| Aadhaar slip | Aadhaar update. No Gazette |
| Genuine change of name | A Central Gazette notice, then each paper |

Central Gazette Process for a Name From Jharkhand, and Where It Fits Hospital Records
No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name File Sent From Jharkhand
Settle the final name first
Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.
Type the proforma in duplicate
Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.
Fill the employment field if it is a change of name
The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then update in the right order
Check every word the day it publishes and download the e-Gazette copy. Then update Aadhaar and PAN, and then your bank account and residence papers, with the original Gazette notification.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | The same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
What a Hospital Record Name Correction Costs
The costs are small: an application, copying charges for records, and perhaps an affidavit. Not a Gazette notice, for a slip. We could not verify what Jharkhand hospitals charge for copies of records, so we print no amount.
| Item | What we can and cannot tell you |
|---|---|
| Correction before discharge | No fee that we know of |
| Corrected papers after discharge | Some hospitals charge. Ask for a receipt |
| Copies of medical records | Not verified. Ask the records department |
| Fresh medical certificate | As the doctor charges |
| Affidavit | Stamp paper, Rs 100 as our safe default, plus the notary's fee |
| Gazette notice | Not needed for a spelling slip. Paid on BharatKosh, revised each year and non-refundable, if ever needed |
| What we charge | Quoted on the call. For telling you which paper to fix, nothing |
