Name Change in Ayushman Bharat Card in Jharkhand
Start with the fact that changes everything about this record. You did not fill in the form your name came from. The scheme covers persons "covered under specific categories of SECC-2011 database of Govt. of India", which means the name on your entry was written down by a survey worker years ago, in a village or a ward, in whatever spelling they used that day. So you are not correcting your own application. You are proving that the person in a decade old survey line and the person holding this Aadhaar are one and the same. That is an identification problem, not a spelling problem, and it needs a different approach. One more thing worth carrying with you: the Government's own frequently asked questions list the documents as the entitlement letter, the ration card and the Aadhaar card "for identification only".
Think of this as matching two records rather than correcting one. The beneficiary list is not something you filled in. The Government's own frequently asked questions describe those covered as persons "covered under specific categories of SECC-2011 database of Govt. of India", so the entry carrying your name was created from a survey. Your Aadhaar was created separately, at a different time, by a different process. When the two do not agree letter for letter, the question an office is really being asked is not how should this be spelt but are these two records about the same human being. Answer that question and you are most of the way there. Two facts are worth carrying with you. First, the same FAQ lists what is needed to avail treatment as the PM or CM entitlement letter, the ration card, and the Aadhaar card "for identification only". Read those last two words carefully, because the Aadhaar is described as a means of identifying you, not as the source of the entitlement. Second, and families are frequently told otherwise, "there is no cap on family size or age of members". And here is what we could not find. That same official FAQ page says nothing at all about what to do when a name is missing or recorded wrongly, and we were unable to open the national beneficiary portal to verify the current correction steps. So this page gives you the verified position, the right way to frame the request, and the grievance route, and prints no form number we have not seen.
Your Ayushman Entry Came From a Survey, Not From an Application You Filled In
Almost every frustration with this card comes from expecting it to behave like a form you submitted. It does not, because you did not submit one.
| The point | What follows from it |
|---|---|
| Those covered are persons "covered under specific categories of SECC-2011 database of Govt. of India" | Your entry began life as a survey record. Somebody walked a village or a ward, asked a household for names, and wrote them down. Nobody checked that spelling against an Aadhaar, because for many households there was none yet |
| The spelling in that line was never yours to choose | Which is why arguing that the card is wrong often gets you nowhere. It is not wrong in the sense a typed form is wrong. It records what was said and heard on a particular day, years ago |
| Your Aadhaar was created separately | Different day, different process, possibly a different spelling of the same name in a different script. Two true records that do not agree letter for letter |
| So the real question at the counter is not about spelling | It is are these two records about the same human being. Frame your request that way and it stops being an argument about letters and becomes a matter of producing the papers that link them |
| In Jharkhand this lands hardest on tribal surnames | Hembrom or Hembram, Oraon or Uraon, a Santali name written by ear. Two forms both correct, one in a survey line and one on an Aadhaar. Our tribal surname spelling correction page sets out how to settle on one |
| "There is no cap on family size or age of members" | Worth carrying with you, because families are often told a member is too old, too young or one too many. The Government's own FAQ says otherwise |
What the Words Identification Only Mean for an Ayushman Card Name
The Government's own frequently asked questions list what is needed to avail treatment, and the wording repays a careful read.
| What the FAQ says | What it means when you are standing at a counter |
|---|---|
| The documents listed are the "PM/CM entitlement letter, Ration Card, Aadhaar Card for identification only" | Read the last two words. Aadhaar is described as a means of identifying you against the list. It is not described as the source of the entitlement, and it is not described as a document your entry must be rewritten to match |
| The ration card is listed alongside it | Often the single most useful paper a Jharkhand household holds for this purpose, because it shows the household and frequently carries the survey era form of the names. Take it. Our ration card name change page deals with that record |
| The entitlement letter is listed first | Where a household has one, it is the paper that says the entitlement exists. Keep it with the ration card rather than loose, and photograph it |
| "There is no cap on family size or age of members" | A quotable answer to two of the commonest things families are wrongly told. Nobody in the household is excluded for being old, young or additional |
| What this does not mean | It does not mean a mismatch will be waved through, and we are not going to pretend otherwise. Identification still has to be satisfied. What it means is that the framing is identify this person, and that is a question you can answer with papers |
| Where the difficulty is genuinely at the Aadhaar end | If the Aadhaar itself carries a spelling nobody in the family uses, settle that first. Aadhaar charges a fee per attempt with no refund and limits how often a name may be updated, so decide once. Our Aadhaar name change page covers it |
What We Could Not Verify About Ayushman Card Name Corrections
We would rather give you a short page you can trust than a long one you cannot. Here is exactly where our knowledge stops, and what to do in each case.
| The question | Our position |
|---|---|
| What we did verify | From a Government PM-JAY frequently asked questions page: that those covered are persons covered under specific categories of the SECC-2011 database, that there is no cap on family size or age of members, and that the documents listed for availing treatment are the entitlement letter, the ration card and the Aadhaar card for identification only |
| The official FAQ is silent on wrong or missing names | We looked specifically. That page does not address what to do if a name is absent from the list or recorded incorrectly. It points to an online eligibility check and a grievance portal and nothing more. So there is no notified correction procedure for us to quote you |
| We could not open the national beneficiary portal | We tried, and it would not open for us while writing this page. So we print no portal step, no button name and no screen sequence, because portals change and we have not seen the current one |
| We could not read the beneficiary identification guidelines | The document we tried was no longer available at the address we found. So we quote nothing from it, including on how an identification mismatch is supposed to be handled at a hospital |
| We have verified nothing specific to Jharkhand | State arrangements can extend or vary coverage. We print no Jharkhand specific rule, office or extension, and you should ask locally rather than assume this page covers a State variation |
| What is still solid, whatever the portal says today | The reframing. Your entry came from a survey, your Aadhaar came from elsewhere, and the task is linking the two. That is true regardless of which screen or form is current, and it is what actually moves these cases |
How to Sort Out an Ayushman Bharat Card Name in Jharkhand
Do this before anybody is ill
That is the whole point of the page. Identification is checked when somebody needs admission, which is the worst possible moment to discover a mismatch. Check it on a quiet evening.
Put the card, the Aadhaar and the ration card together
For every member of the household, not just the person who usually deals with papers. Read each name letter by letter.
Decide which record is actually the odd one
If the Aadhaar agrees with the bank, the school records and everything else, the Aadhaar is right and the list entry is the survey era one. That changes what you ask for.
Correct the Aadhaar only if it is genuinely wrong
Not to match an old list. Aadhaar charges a fee per attempt with no refund, so if you do go, settle the family spelling first and go once.
Gather what links the two records
The ration card, the entitlement letter if you have one, and any older document carrying the survey era spelling. This chain is your case.
Ask at an empanelled hospital's help desk
They deal with identification daily and will often tell you in minutes what is needed. This is usually the fastest and cheapest answer available to you.
Frame the request as identification, not correction
Say that the list entry and the Aadhaar are the same person and that you have brought what shows it. It is the truth and it is the version an office can act on.
Ask what is needed, in writing where you can
We print no form number or portal step, because we could not verify the current ones. Let the office's own answer be your list.
Use the grievance route if an office will not act
The scheme publishes one. A written record of what you asked and when is what makes that route usable, so keep every acknowledgement.
Check every member is on the record
Because there is no cap on family size or age of members. If somebody has been left off or told they are excluded for age, that is worth taking up with the quotation in hand.
Bring the household's other records into line
The ration card, the caste certificate and the children's school records usually carry the same spelling split.
Photograph the card and keep it reachable
Somewhere the whole family can find it, not in one person's pocket. The people who need it are often not the person who arranged it.
Where a Hindi form of a name sits against an English one elsewhere, our Hindi to English name spelling difference page sets out how to choose, and where a whole family carries the same split, our one public notice for several certificates page explains how that is handled together.
Central Gazette Process for a Name From Jharkhand, and Where It Fits a Scheme Record
No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name File Sent From Jharkhand
Settle the final name first
Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.
Type the proforma in duplicate
Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.
Fill the employment field if it is a change of name
The adult proforma asks for the post of the applicant and the office or company name. A homemaker or a daily wage worker should still complete it rather than leaving it blank, because a blank field brings the file back.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then update in the right order
Check every word the day it publishes, download the e-Gazette copy, and then work through Aadhaar first, the ration card next, and the health card entry after them.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | The same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
What an Ayushman Bharat Card Name Correction Costs in Jharkhand
| Item | Cost | Time |
|---|---|---|
| Checking every household member's entry against their Aadhaar | Nothing, and it is the only step that really matters. The card, each Aadhaar and the ration card, on one evening, before anybody is ill | One sitting |
| Asking at an empanelled hospital's help desk | Nothing. They handle identification every day, and in our experience this is the fastest route to a correct answer about what your particular case needs | One visit |
| Correcting Aadhaar, only where it is genuinely wrong | Aadhaar charges a fee per attempt with no refund and limits how often a name may be updated. This is the only charge we would expect a household to meet, and only after deciding the spelling once | Set by the centre |
| The card correction itself | We print no figure, no form number and no portal step, because the official FAQ we read is silent on name corrections and the national beneficiary portal would not open for us. Ask the help desk or the local body | Set by the office. We print no timeline we have not verified |
| The grievance route, where an office will not act | Nothing to use. The scheme publishes a grievance route, and what makes it usable is your own written record of what you asked and when | Set by the scheme |
| Affidavit, where an office asks for one | Rs 100 non-judicial stamp paper and the notary charge. Do not buy one because somebody in a corridor suggested it. Buy it when an office asks, preferably in writing | The same day |
| Government printing charge, only in a genuine change of somebody's own name | The guidelines print Rs 1,100 for an adult change of name and Rs 1,400 for a public notice, for FY 2016 to 2017, each with the caveat that the charge varies each financial year and from case to case. What BharatKosh shows on the payment screen the day you pay is the figure that counts | Part IV publishes weekly, so allow several weeks from posting |
| Our charge | Quoted on your papers before you pay anything, and confirmed in writing. We print no service price on any page of this site, and on a health card we expect to tell most families there is nothing for us to charge for | |
Myths About Ayushman Bharat Card Name Corrections
| What people are told | What is actually so |
|---|---|
| Your name on the card is wrong because somebody made a mistake | Usually nobody did. Those covered are persons "covered under specific categories of SECC-2011 database", so the entry began as a survey record written down years ago. It is not a wrong form, it is an older record, and that changes how you should ask |
| Change your Aadhaar so it matches the card | Often exactly the wrong direction. If your Aadhaar agrees with your bank, your ration card and everything else, changing it to match one old survey line puts your strongest document out of step with all the others. Ask instead what is needed to identify you against that entry |
| A gazette notification will fix the card | It will not. A gazette is about a person's name. It does not add a household to the beneficiary list and it does not resolve an identification question at a hospital desk. Worse, this is often sold to a frightened family in a hospital corridor, when treatment cannot wait weeks for a publication |
| Elderly parents or extra children are not covered | The Government FAQ says plainly that "there is no cap on family size or age of members". If somebody has been told a member is excluded for age or number, that sentence is worth quoting |
| Aadhaar is what makes you eligible | The FAQ lists the documents as the entitlement letter, the ration card and the Aadhaar card "for identification only". Aadhaar is described as the way you are identified against the list, not as the source of the entitlement |
| Leave the ration card at home, the Aadhaar is enough | Take the ration card. It is listed in the official documents, it shows the household, and in Jharkhand it very often carries the survey era form of the names, which is exactly the link you are trying to show |
| Sort it out when somebody needs admission | The worst possible plan, and the reason this page exists. Identification is checked at the moment of admission, when the family is least able to argue and most likely to be sold something expensive. Check the card on a quiet evening instead |
| The step by step guides online are official | Most are not. Nearly everything ranking on this subject is a private website designed to look official. A Government page address ends in a Government domain, and on a health card a wrong instruction costs a family a trip they made instead of sitting with somebody in hospital |
| There is a standard name correction form for this card | If there is, we could not find it, and we looked. The official FAQ page we read is silent on missing or wrongly recorded names, and the national beneficiary portal would not open for us. So we print no form number, and we tell you to ask the hospital help desk or the local body |
| Two spellings of a tribal surname mean one of them is wrong | Not necessarily, and treating it that way sends you down the wrong road. Both forms are often in genuine use. Settle on one and carry it everywhere, using our tribal surname spelling correction page |
Key Terms in an Ayushman Bharat Card Name Correction
| Term | What it means in practice |
|---|---|
| Beneficiary list | The list of persons covered, drawn from specific categories of the SECC-2011 database. Not a list you applied to join |
| SECC-2011 | The socio economic survey your entry most likely originates from. A survey record, written down by an enumerator, years ago |
| Entitlement based | Coverage that follows from being in a listed category rather than from filling in an application. The reason this record behaves unlike every other one on this site |
| Identification only | The words the official FAQ uses about the Aadhaar card. It identifies you against the list; it is not described as the source of the entitlement |
| Entitlement letter | The PM or CM letter listed among the documents for availing treatment, where a household has one. Keep it with the ration card |
| Ration card | Listed among the documents, shows the household, and often carries the survey era form of the names. The most under used paper in these cases |
| No cap on family size or age | The Government FAQ's own words. A quotable answer when a member is said to be excluded for being old, young or additional |
| Empanelled hospital | A hospital where the scheme can be used. Its help desk handles identification daily and is usually the fastest source of a correct answer |
| Identification | Establishing that the person present is the person in the entry. The actual task on this page, as against correcting a spelling |
| Grievance route | The complaint channel the scheme publishes, for when an office will not act. Usable only if you kept a record of what you asked and when |
| Survey era spelling | The form of a name as recorded at the time of the survey. Often different from an Aadhaar without either being wrong |
| Household | The unit the entry describes. Check every member, not only the person who handles the papers |
Ayushman Card Name Help Across Jharkhand
The offices that matter here are close to you: the empanelled hospital's help desk, the Common Service Centre and the local body. None of those is something we can stand in front of, and we would not pretend otherwise. What we do is read the card against every household member's Aadhaar and the ration card, tell you which record is actually the odd one and whether an Aadhaar correction is genuinely needed or would be a mistake, assemble the papers that link the survey entry to the person, and tell you how to put the request so it is treated as identification rather than a complaint. That is done on your scans over WhatsApp, for every district, and for most families it costs nothing.
District pages are linked from the services page.
Send the card and the Aadhaar and we will read both
Send a photograph of the Ayushman card or the entry as it appears, the Aadhaar of every member of the household, and the ration card. Send the entitlement letter if the family has one. Tell us your district, and tell us straight away if somebody is currently admitted or about to be, because that changes what we say first. Our first reply tells you which record is actually the odd one, whether an Aadhaar correction is genuinely needed or would be a mistake, which papers link the survey entry to the person, and the exact words to use so the request is treated as identification rather than a complaint. For most families there is nothing for us to charge for, and that is what our first reply will say. Details on the contact page.
