Public Notice Proforma Format for a Wrongly Written Name
Everything that gets typed in a public notice file, in one place: the specimen proforma with its blanks, the certification line the Department prints inside it, the witness block, the soft copy rule that trips people up, and the forwarding letter. The wording here is the public notice format, for a name that has been wrongly written. It is a different form from the change of name proforma, which opens with hitherto known as and even asks for your employer's name. Use the wrong one and the file comes back with the fee already spent. Both are shown side by side below so you can see at a glance which is yours.
The public notice proforma is the Department of Publication's specimen format for declaring that a name has been written wrongly in records. Its opening line reads that it is for general information that the applicant, son, daughter or wife of a named person and residing at a named address, declares that the name of himself, his father, his mother, his minor son, his minor daughter, his wife or his husband has been wrongly written as a stated version, and that the actual name is or are the correct version or versions respectively, which may be amended accordingly. That word respectively is worth noticing, because it is how one notice can carry several names or several documents at once. The form then carries a certification line saying the applicant has complied with the other legal requirements in this connection, followed by a witness block asking each of two witnesses for a signature, a full name, an address and a mobile number. It goes in as two original typed copies, signed by the applicant in his own correct name. Alongside it travel an undertaking, the original newspaper, two self-attested photographs with identity proof, an attested photocopy of the document carrying the error, a soft copy in MS Word without the witness portion and with the name typed where the signature would be, a signed certificate that the hard and soft copies match, and a forwarding letter to the Controller of Publication with the printing charges. One rule catches people out and appears on no competitor page: the documents must not be older than one year. From Jharkhand the whole set goes by post or courier to Civil Lines, Delhi 110054, because no district in the state has a gazette counter.
The Public Notice Proforma for a Wrongly Written Name, Line by Line
Here is the Department's specimen wording with the blanks as the form carries them. Read it once as a whole, then take the parts one by one.
Blank One: Your Own Name
Your correct name goes here, the version you want every record to carry at the end. Not the misprinted one. This is the name you will also sign in at the bottom, and it should match the identity proof you attach.
Blank Two: Father, Husband or Guardian
The son, daughter or wife line, with the named person. In Jharkhand this is where a father's name is written, and its spelling has to match the one on the identity papers in the file. If his name is itself the thing being corrected, say so in the declaration part rather than quietly writing the corrected version here.
Blank Three: The Full Residential Address
With the pin code. It has to match the address on the undertaking and on the identity proof, and it is the address that appears in the published notice. A Ranchi address on the form and a Bokaro address on the Aadhaar is the sort of mismatch a counter picks up in seconds.
Blank Four: Whose Name Carries the Error
This is the part that makes the form far more useful than people realise. The printed list covers your own name, your father's, your mother's, your minor son's, your minor daughter's, your wife's and your husband's. So a parent can publish about a child's record, and an adult can publish about a parent's name printed on his own certificate.
Blank Five: The Wrong Version, Quoted Exactly
Reproduce the error, do not describe it. Copy it from the document letter for letter, including a missing letter, a doubled letter, a joined word or a dropped surname. "Wrongly written as RAJIB KUMAR MEHTA" is usable. "Spelling mistake in my name" is not, because an office has nothing to match against its own record.
Blank Six: The Correct Version, and the Word Respectively
The actual name goes here, and the form says is or are and respectively. That plural is deliberate. If you list two or three wrong versions in order, you list the matching correct versions in the same order, and one notice covers them all. This is how a father publishes once for three children, and how one notice covers a marksheet, a certificate and a caste certificate together.
The Undertaking Format and the Certification Line
The proforma does not travel alone. The Department's list for a public notice begins with an undertaking duly signed by the applicant, and the proforma itself carries a certification line inside it. Two different things, and both have to be right.
The certification inside the proforma
The form carries the printed line that it is certified that the applicant has complied with the other legal requirements in this connection. You are not writing this sentence, it is part of the specimen, but you are signing under it. It is the reason the newspaper has to be published before the file goes, not after.
The undertaking as a separate paper
Signed by the applicant, carrying the same names and the same address as the proforma, word for word. It says the statement is true and correct to the best of your knowledge and belief, and that if any legal issue arises at any stage you will be personally responsible for it. That last clause is why a false or careless notice is a bad idea, not merely a wasted fee.
What Must Match Across the Two Papers
| The detail | Why a mismatch matters |
|---|---|
| The wrong version | If the proforma and the newspaper quote the error differently, the file shows two different errors and the counter cannot tell which one is being declared |
| The correct version | Spelt identically everywhere, including in the undertaking. A single letter apart and you have published one name and sworn another |
| The father's or husband's name | It appears in the proforma, in the newspaper matter and on the identity papers. All three have to agree |
| The address | Same on the proforma, the undertaking and the identity proof, with the same pin code |
| The list of documents | The certificates named in the proforma should be the ones named in the newspaper matter too |
The Witness Block and Who Signs a Public Notice Proforma
The witness block sits at the foot of the proforma and asks each of two witnesses for a signature, a full name and an address, along with a mobile number. Both original copies carry both signatures, and each witness's self-attested identity and address proof goes into the file.
You sign in your own correct name
A public notice declares that a record is wrong, so nothing of yours is changing and you sign as yourself. This is the opposite of the change of name category, where the Department asks for the copies to be signed in the existing old name.
Two witnesses, with mobile numbers
Real people who can be reached. The mobile number is asked for on the form, so a witness whose number is out of service is a weak point in the file.
Each witness's proof goes in
Self-attested identity and address proof for both. A witness with no proof attached is a witness the file cannot rely on.
The Order to Prepare a Public Notice File In
Settle the correct spelling first
Letter by letter, and normally the version your live documents already agree on. Everything below is typed from this one decision, so changing your mind afterwards means starting again.
Write the two lists
Every wrong version in order, and every correct version in the same order. This is what the word respectively in the proforma is for, and it is the difference between one fee and three.
List the documents with roll numbers
Each certificate named, with its roll number and year where it has one. This list goes into the proforma and into the newspaper matter.
Type the proforma in duplicate
The public notice format, not the change of name one. Leave the certification line as printed. Sign both copies in your own name and get both witnesses to sign both.
Prepare the undertaking
Same names, same address, word for word. We put it on Rs 100 non-judicial stamp paper with a notary as the safe default.
Publish and keep the whole paper
One of the daily local leading newspapers, the matter showing the details of the case. Keep the entire original paper, not a cutting.
Prepare the soft copy the way it is asked for
MS Word, without the witness portion, with the name typed where a signature goes. Then the certificate signed by you that the hard and soft copies are the same.
Check the dates, then post it yourself
No paper older than one year. Attach the photographs, identity proof, the attested copy of the wrong document and the BharatKosh receipt, write the forwarding letter, photocopy everything, and post it in your own name.
Public Notice Proforma Compared With the Change of Name Proforma
These are two different forms in two different guideline documents. Seeing them beside each other is the fastest way to be sure you are on the right one.
| Point | Public notice, wrongly written | Change of name, adult or major |
|---|---|---|
| Opening words | It is for general information that I ... declare that name ... has been wrongly written as | I hitherto known as (old name) ... have changed my name and shall hereafter be known as (new name) |
| Who it can speak about | You, your father, mother, minor son, minor daughter, wife or husband | The applicant only |
| Employment details | Not asked for | Asked for. The form carries the post of the applicant and the office or company's name |
| Who signs, and in which name | The applicant, in his own correct name | The applicant, in the existing old name |
| Several names at once | Yes, through the is or are ... respectively construction | One applicant, one change |
| The document carrying the error | An attested photocopy of the connected legal document goes in the file | Not applicable, nothing is wrong anywhere |
| Newspaper | Published in one of the daily local leading newspapers, showing the details of the case | Advertised in one of the daily local leading newspapers, stating the father's or husband's name along with the residential address |
| Printing charge in the guidelines | Rs 1,400 for the financial year 2016 to 2017, varying each financial year and from case to case | Rs 1,100 for the same year, and Rs 3,500 for an Indian living abroad |
Central Gazette Public Notice Process From Jharkhand, Step by Step, Once the Proforma Is Typed
No district in Jharkhand has a gazette counter, so the finished set goes from Ranchi, Jamshedpur, Dhanbad, Bokaro or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state route to weigh against this one.
The Eight Steps of a Central Gazette File Sent From Jharkhand
Fix the correct spelling before typing
The Gazette publishes what is written. One wrong letter means a corrigendum, a second fee and a second wait, so the spelling is settled once and then copied into every paper.
Pick the category, then its proforma
The Department runs twelve separate guideline categories, each with its own form. Public notice for a wrongly written name, change of name for an adult, a different one for a minor, for a maiden surname, for a different surname. Download only the one you need.
Type the proforma in duplicate
Two originals, the error quoted exactly, the correct versions in the same order, every document named. Signed by you in your own name and by two witnesses with their addresses and mobile numbers.
Sign the undertaking
Same names and address as the proforma, word for word, with the clause accepting personal responsibility if a legal issue arises later.
Publish and keep the original paper
One of the daily local leading newspapers, the matter showing the details of the case, with the paper's name and date visible. The whole original paper goes in the file.
Pay only through BharatKosh
The Government fee goes through BharatKosh, the Non Tax Receipt Portal, and nowhere else since October 2018. Print the e-receipt. No agent can pay it on another portal.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, in your own name. Photocopy the whole set first, because nothing sent in comes back.
Read the notice, then download it
Check every letter and every document name the day it publishes, note the page and issue number, and download the e-Gazette copy of that Part IV issue to keep permanently.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the public notice format; the wrong version quoted exactly; the correct versions in the same order; the relationship line where the error is in a parent's name; the father's or husband's name; the full address with pin code; your signature in your own name; two witnesses complete with signature, full name, address and mobile number |
| 2. The undertaking | Signed by the applicant; the same names and address as the proforma; the declaration that the statement is true and correct to the best of your knowledge and belief, with personal responsibility accepted if a legal issue arises |
| 3. The original newspaper | The original paper, not a cutting; the paper's name and date of publication visible; the details of the case shown; the same spelling as the proforma in every place; and dated within the last year |
| 4. Photographs, identity proof and the connected document | Two self-attested passport photographs; identity and address proof matching the proforma address; both witnesses' proofs; and the attested photocopy of the document that carries the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, name typed in place of the signature; the signed certificate that hard and soft copies are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt showing payment to the Department of Publication |
The posting detail from Jharkhand is on the sending the file to Delhi page, the payment walkthrough on the BharatKosh page, the download on the download page, the rejection reasons on the rejection page, and the fix for a published error on the corrigendum page.
The Soft Copy, the Forwarding Letter and the One-Year Rule
Three requirements sit outside the proforma itself, and between them they account for a large share of returned files.
The soft copy, prepared differently
A CD carrying the print matter in MS Word, without the witness portion, and with the name typed in place of the signature. People copy the signed form into Word exactly as it stands, witnesses and all, and the file comes back. Prepare the soft copy as a separate clean document.
The certificate about the soft copy
A short certificate signed by you declaring that the contents of the hard copies and the soft copy are the same. It is a separate signed paper, not a line inside the proforma.
The forwarding letter
Addressed to the Controller of Publication, sent along with the documents, the printing charges and the photo identity proof. Put a telephone number and an email address on it, because that is how the Department reaches you if something is short.
Public Notice Printing Charges and Time From Jharkhand
| Item | What to expect |
|---|---|
| What the guidelines print, by category | All for the financial year 1 April 2016 to 31 March 2017, each adding that it varies each financial year and from case to case. Public notice Rs 1,400. Change of name, adult or major, Rs 1,100. Change of name and gender, Rs 1,400. Indian living abroad, Rs 3,500. |
| What applicants commonly pay now | Commonly around Rs 1,350 for a public notice with the undertaking, Rs 1,100 for an adult change of name and Rs 1,700 for a minor. These are revised each financial year, so read the amount on BharatKosh on the day you pay rather than trusting any website, this one included |
| Extra names or extra matter | The Department publishes a separate note on details where additional charges are applicable. Several names or several documents in one notice can raise the amount, which is what varying from case to case means, and it is still far cheaper than publishing separately |
| How it is paid | BharatKosh only, since 1 October 2018. Non-refundable, whether the notice publishes or the file comes back |
| Newspaper cost | Paid to the paper, not the Department. A public notice showing the details of the case runs longer than a one-line change of name, so it costs more than people expect, and listing several documents lengthens it further |
| Undertaking and notary | Rs 100 non-judicial stamp paper is the safe default in Jharkhand, plus the notary's charge |
| Time to publish | Part IV publishes weekly. Allow several weeks from posting, and more if the file travels back and forth. The six-month window to make good a shortcoming is the outer limit before a fresh application is needed |
| Our charge | Quoted on your papers before you pay anything, and confirmed in writing. We print no service price on any page of this site |
The whole journey cost is on the name change cost page, and the gazette stage on its own is on the time and fees page.
Myths About the Public Notice Proforma Format
"One gazette proforma fits every case"
There are twelve guideline categories, each with its own form. Public notice, adult change of name, minor, maiden surname, a different surname, name and gender, religion, adoption and more. The category is chosen first, the form second.
"I can handwrite the form"
The wording asks for the proforma duly typed, in duplicate. A handwritten form invites a second look, and two handwritten copies will never match each other exactly.
"A photocopy of the signed form is the second copy"
It is not. Two original typed copies, both signed by you and by both witnesses in ink.
"The soft copy is just the same file"
It is not. The CD carries the print matter without the witness portion, in MS Word, with the name typed where the signature goes. Copying the signed page straight into Word is a common reason for a return.
"Old papers are fine, nothing expires"
The guidelines say the documents must not be older than one year. An old newspaper page or a stale identity copy can fail the file on date alone.
"I can write in various documents and leave it there"
Name each document, with its roll number and year where it has one. A document not named is a document the notice does not cover.
"Two witnesses are a formality"
The form asks each for a signature, a full name, an address and a mobile number, and their identity proofs go in the file. Pick people who can be reached.
"I sign in the wrong spelling since that is what the records say"
No. On a public notice you sign in your own correct name. Signing in the misprinted version is an agent's error, and it makes the file contradict itself.
"An agent can submit the form at the counter"
The Department states only personal submission or post and courier are entertained, and names agents and advocates as not permissible. You post it yourself, in your own name.

Proforma Preparation Across Jharkhand
The form is typed from your scans, so the district does not matter. Send the documents over WhatsApp from anywhere in the state and the wording comes back to you ready to print, sign and post.
District pages are linked from the services page.
Send your papers for a format check
Photograph the document carrying the wrong name, one document carrying the correct version, and your Aadhaar. The first reply tells you which of the Department's categories applies, whether the issuing office can fix it without any gazette at all, and if a notice is the route, the exact proforma wording with every wrong version quoted and every document named. If more than one person in the family carries the same error, say so, because the form is built to cover several names in one notice. Details on the contact page.
