Name Change in ESIC Records in Jharkhand
A wrong name in your provident fund record costs you money, later. A wrong name in your ESIC record costs you treatment, today. That is the whole difference and it is why this page is written differently from the rest. Your ESIC entry is not only about you. It carries the family members who are entitled to medical care on your card, and a name that does not match is discovered at a dispensary or hospital counter, usually by a relative, usually when somebody is unwell. The other hard fact is that the gateway is your employer. So the single best piece of advice on this page is simple: check it and fix it while you are still in the job, not after you leave.
Treat this as two jobs, not one. The first job is your own name. Open your ESIC record and read the name letter by letter against your Aadhaar, spacing and full stops included. A difference here is almost always a spelling drift, an initial or a name order question rather than a different name, which means it is a correction and not a change of name, and no gazette is published for it. The second job is the one almost everybody forgets, and it is the one that actually causes the trouble: the family members listed on your ESIC record. Those are the people entitled to treatment on your card, and if a spouse, child or parent is recorded with a wrong name, or was never added at all, the problem surfaces at a dispensary or hospital counter at the worst possible moment. Check every name on that list, not just your own. The route runs through your employer. ESIC put in place an online system under which employers submit requests to update an insured person's own and family particulars through the employer portal, by a circular reported as dated 22 December 2022. ESIC also runs a separate insured person login and an Aadhaar seeding exercise covering both insured persons and their dependants. We have not read the circular or the user manual ourselves, so treat this page as the shape of the process, and confirm what your own portal offers on the day. And the practical rule that matters more than any of it: do this while you are still employed, because once you leave, the person who has to file the request is somebody you no longer see.
Why an ESIC Name Error Is Not the Same Problem as a Provident Fund One
People treat these two as the same errand because both are employment records. They behave completely differently, and the differences decide how urgently you should act.
| Provident fund | ESIC |
|---|---|
| What is at stake is money, and you find out at a claim or transfer | What is at stake is medical treatment, and you find out at a dispensary or hospital counter, usually when somebody is already unwell |
| The record is about you alone | The record carries your family members as well, because they are entitled to care on your entitlement. Most real failures are on their names, not yours |
| Since the simplification announced in January 2025, many members can update their own profile with an Aadhaar OTP | The route runs through the employer portal, under the online edit system ESIC put in place, so in practice your employer is the gateway |
| You can usually sort it out after you leave, slowly, if you must | Sorting it out after you leave is far harder, because the person who files the request is an employer you no longer work for. Do it while you are in the job |
| A delay costs you waiting | A delay can cost you at the exact moment care is needed, which is why this page treats it as urgent rather than administrative |
| The anchor is your Aadhaar, because the UAN is Aadhaar seeded | Aadhaar matters here too. ESIC runs an Aadhaar seeding exercise covering insured persons and their dependants, so the names on your record and your family's Aadhaar should agree |
The Family and Dependant Names on Your ESIC Record
Your own name is the part you would think to check. The family list is the part that actually fails, because nobody has looked at it since an office typed it, and because the people on it are the ones who turn up at a counter needing care.
| What you may find on the list | Why it matters and what to do |
|---|---|
| A family member is missing entirely | The commonest and the most serious. A person who is not on the record cannot be identified against it at a counter. This is not a name correction at all, it is an addition, and it should be requested as one, with the relationship evidence |
| A spouse name recorded before marriage, or in a maiden form her documents no longer carry | Extremely common in this state, where many women take a husband surname at marriage. Her ESIC entry should match the documents she actually holds today. The position on Kumari and Devi forms is on our Kumari or Devi to the husband surname page |
| A child recorded with a pet name, such as Raju, Munna or Guddu | Exactly the same trap as a school admission register. The child's own documents will carry the proper name and the ESIC record will not, and the two are compared at a counter. Our short name and full name page explains which route that needs |
| A dependant parent name spelt differently from their own papers | Old records and old spellings. Bring the ESIC entry to whatever their own documents carry today, not to what the family says at home |
| A relationship recorded wrongly, for example a daughter in law shown as a daughter | Not a name question at all, and it should be raised separately and plainly. Do not bundle it into a spelling request, because the two need different evidence |
| Everything looks right on the printout | Check it against the Aadhaar of each person named, not against memory. ESIC runs an Aadhaar seeding exercise covering insured persons and their dependants, so the two should agree |
How an ESIC Name Correction Actually Moves, and What We Could Not Verify
Here is what we can tell you honestly, separated from what we cannot. This distinction matters more on this page than on most, because the procedure is published as circulars and user manuals that are revised, and a great deal of confident advice online is copied from old versions of them.
| What we can say | On what basis |
|---|---|
| ESIC put in place an online system for employers to submit requests to update and edit an insured person's own and family particulars through the employer portal | From a circular reported as dated 22 December 2022. We have not read the circular itself, and we say so rather than pretend otherwise |
| ESIC operates a separate insured person login and employer login | Both are offered on the ESIC website itself. What each login allows you to edit is the thing to check on the day, because it changes |
| ESIC runs an Aadhaar seeding exercise covering existing insured persons and their dependants | ESIC's own regional offices publish material on Aadhaar seeding for insured persons and dependants. This is why the names on your ESIC record and on each person's Aadhaar should agree |
| What we will not print: which exact fields you may edit yourself, how many times a field may be changed, what approval each request needs, or how long any of it takes | We have not read the circular or the user manual. Anyone stating those details with confidence has probably copied them from a summary of an older version |
| What we will not print: the wage limit for ESI coverage, or any ESIC branch office or hospital address | Thresholds are revised and addresses move. Check the current position on esic.gov.in, or send us your papers and we will tell you which office your employer is attached to |
How to Correct a Name in ESIC Records in Jharkhand, Step by Step
Do it on a day when nobody is ill
This is a real step, not a remark. Every family that comes to us is already at a counter with a sick relative. The whole check takes ten minutes on a quiet evening.
Read your own name against your Aadhaar
Letter by letter, spacing and full stops included. Then work out which kind of difference it is, because a spelling drift, a short form and a name order question each need different wording.
Read the whole family list, name by name
With each person's Aadhaar in front of you. Look for a wrong spelling, a maiden name, a child's pet name, and above all for somebody who is missing from the list altogether.
Decide what each entry should match
Write it down per person. The first question anybody asks is which document the record should agree with, and families almost never arrive with that decided.
Log in and see what your portal offers
Rather than trusting any article, including this one, about which fields can be edited and by whom. Procedures of this kind are revised and online summaries go stale.
Ask your employer in writing, today
To the compliance or human resources desk, with the insurance number, employer code, the entry as it stands and as it should read, and the supporting document. Keep a copy with the received stamp.
If you have left the job, go to the branch office
With whatever service evidence you hold and the identity documents for each person concerned. There is no employer to file for you, so build the file properly before you go.
Check the record again after the update
Open it and read every name once more, yours and the family's. Finding a leftover difference now is a minute. Finding it at a hospital counter is a different kind of day.
Where the underlying difference is a spelling matter, start at name spelling correction. The provident fund side of the same problem, which has a different route and a different anchor, is on our name change in EPFO and UAN records page, and the Aadhaar route itself is on our name change in Aadhaar card page.
Central Gazette Process for a Name From Jharkhand, and Where It Fits an ESIC Case
No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name File Sent From Jharkhand
Settle the final name first
Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.
Type the proforma in duplicate
Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.
Fill the employment field if it is a change of name
The adult proforma asks for the post of the applicant and the office or company name, a field the public notice form does not have. Salaried applicants leave it blank and the file comes back.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then update the records
Check every word the day it publishes, download the e-Gazette copy of that Part IV issue, and then work through Aadhaar first and the ESIC record after it.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | The same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
What an ESIC Name Correction Costs in Jharkhand
| Item | Cost | Time |
|---|---|---|
| The correction request itself, filed while you are employed | No charge that we are aware of. The real cost is getting it filed properly and once, with the insurance number, the entry as it stands and as it should read, and the supporting document. We do not print a portal charge, because we have not verified one | We do not print a processing time we have not verified. Ask the desk that files it |
| Adding a family member who is missing | An addition rather than a correction, needing relationship evidence. Again no charge we are aware of, and again worth doing before it is needed rather than during | Set by the office handling it |
| After you have left the job | No fee as such, but the cost becomes time and persistence, because there is no employer desk to file for you. This is the honest reason to do it early | Weeks, and longer where a former employer has to be traced |
| Fixing the Aadhaar first, where the Aadhaar itself is the wrong one | Aadhaar charges a fee per attempt with no refund and limits how often a name may be updated. Go once, with the right evidence | Set by the centre |
| Board or school correction, where a certificate is the evidence and it is itself wrong | The board or institution own charge, plus Rs 10 for an RTI application where a register page has to be obtained that way. We never print the JAC correction fee, because the figures circulating online belong to other boards | Set by the board |
| Affidavit, where an office asks for one | Rs 100 non-judicial stamp paper and the notary charge. Two originals, because the office keeps one | The same day |
| Government printing charge, only in a genuine change of name | The guidelines print Rs 1,100 for an adult change of name and Rs 1,400 for a public notice, for FY 2016 to 2017, each with the caveat that the charge varies each financial year and from case to case. What BharatKosh shows on the payment screen the day you pay is the figure that counts | Part IV publishes weekly, so allow several weeks from posting |
| Our charge | Quoted on your papers before you pay anything, and confirmed in writing. We print no service price on any page of this site | |
The truthful summary is that an ESIC correction is rarely expensive in money. It is expensive in timing. Filed from inside the job it is a short errand. Filed after you leave, or at a counter with a sick relative waiting, it becomes the slowest version of itself. Our name change cost page sets out the line items in full.
Send the papers and we will check the whole family list
Send your ESIC record showing the name and the family member list, your Aadhaar, and the Aadhaar of every person named on that list. That last one is the part families leave out and it is the part that matters, because the names that fail at a counter are usually a spouse, a child or a parent rather than yours. Tell us whether you are still in the job or have left, because that changes the whole route. Our first reply tells you which entries actually differ, what kind of difference each one is, whether anybody is missing from the list altogether, and exactly what to put in the request so it can be acted on the first time. Myths About ESIC Name Correction
| What people are told | What is actually so |
|---|---|
| You need a gazette notification to correct a name in ESIC | You do not. An ESIC record is corrected, not published. A gazette matters only where you are genuinely changing your name across all your records, and even then the ESIC entry simply follows afterwards |
| Only your own name matters | The family member names matter more, because they are the people who present themselves for treatment. A missing or wrongly spelt dependant is the commonest real failure and the one nobody checks |
| It can wait until you need it | Needing it means somebody is unwell at a counter. That is the point at which every option is the slow one. Check it on a day when nobody is ill |
| You can sort it out after you leave the job | You can try, but the gateway is your employer, and after you leave that is a company with no reason to hurry, often a contractor that no longer exists. Do it while you are still employed |
| A missing family member is a name correction | It is an addition, not a correction, and it needs relationship evidence rather than a spelling argument. Asking for the wrong thing is why these requests come back |
| Any close enough name will be accepted at the counter | The comparison is done on the record against an identity document. An initial, a maiden surname or a child's pet name is enough to stop it, which is why the check has to be letter by letter |
| Whatever the internet says about the ESIC portal is current | Much of it is a summary of a circular the writer never opened, describing a portal that has since changed. Log in and look, and ask the desk that would file it what they can actually do |
| Your ESIC and provident fund corrections are the same errand | They are not. Different gateway, different urgency, and ESIC covers your family while the provident fund covers only you. The provident fund route is on our EPFO and UAN page |
Key Terms in an ESIC Name Correction
| Term | What it means in practice |
|---|---|
| ESIC | The Employees State Insurance Corporation, which provides medical and related benefits to insured employees and their families |
| Insured person | The employee covered under the scheme. Often written as IP on forms and portals |
| Insurance number | Your identifying number under the scheme, and the first thing to quote in any request |
| Family particulars | The family members recorded against your entitlement, who may receive care on it. The part of the record this page most wants you to read |
| Dependant | A family member entitled through you, such as a spouse, child or dependent parent, each recorded with a name and a relationship |
| Employer portal | The login through which an employer files requests about its employees, including the online edit of insured person and family particulars |
| Insured person login | The separate login offered to the employee. What it allows you to change is the thing to check on the day, because it is revised |
| Aadhaar seeding | Linking the record to Aadhaar, which ESIC runs for insured persons and dependants alike, and the reason the names should agree |
| Branch office | The ESIC office your employer is attached to, which is where a former employee usually has to go. Look it up on esic.gov.in rather than assuming the nearest one |
| Addition | Putting a family member on the record who is not there. A different request from a correction, needing relationship evidence |
| Correction | Putting an existing entry right against documents already held. Cheap, publishes nothing, and what almost every ESIC name case actually is |
| Received stamp | The office acknowledgement on your own copy of the request. Without it, a request never happened |

- The record carries your family, and their names fail more often than yours.
- Check for anybody who is missing from the list altogether.

- The employer portal is the gateway, so file it while you are still employed.
- Read every name against that person's own Aadhaar, letter by letter.
- No gazette is published for an ESIC spelling or a family member name.

Most people check only their own name. The entry that fails at a counter is usually a spouse, a child or a parent, and sometimes it is somebody who was never added at all.
