This is the one subject on this website where the law is already written on your side • Three times over, in an Act and in rules

Name Change in Disability Certificate and UDID Card in Jharkhand

Most pages on this website describe records where the rules are silent about names, where nothing can be edited, and where nobody will say who is responsible. This subject is the opposite, and it is worth knowing that before you walk into any office. The Rights of Persons with Disabilities Act 2016 says that "The certificate of disability issued under this section shall be valid across the country", and it gives a right of appeal: "Any person aggrieved with decision of the certifying authority, may appeal against such decision." The rules made under it name the application form, give the medical authority one month to issue the certificate, require written reasons on a named form where an application is refused, and say plainly that "No other proof of residence shall be demanded from the applicant who has aadhaar or aadhaar enrollment number." You are not asking for a favour here. You are asking for something the law already provides.

1Month, the time the rules give for issuing the certificate
1Country, because the certificate is valid across all of it
2Forms the rules name, one to apply and one for reasons
0Extra residence proofs they may demand if you give Aadhaar
Quick answer

Ask for the existing record to be corrected. Do not apply again under the corrected name. That one sentence prevents the commonest and most damaging mistake on this subject. The Unique Disability ID system is described by a Government of India institute as an "end-to-end integrated system for Issuance of Universal ID & Disability Certificates for Person with Disabilities", built for "Online availability of data of Person with Disabilities across country through a centralized web application" and expressly for "Non-duplication of PwDs data". Non-duplication means a second application in a slightly different name is precisely what the system is designed to catch. A person who applies afresh to escape a spelling does not end up with a corrected record; they end up with two records and a much harder problem. The same official description lists "Online renewal and update of information by Person with Disabilities" among the system's own functions, which tells you that updating your information is a designed part of it rather than an exception somebody has to be persuaded to allow. And a correction of a spelling is not a fresh assessment of disability. The Act's assessment provisions are about assessing the extent of a specified disability. A name is a particular, not an assessment, and your application should say so in those words. Keep the certificate you already hold, do not surrender it, and read the section below on what the Act and the rules already give you, because it is more than most people are ever told.

What the Disability Rights Act and Rules Already Give You in Writing

Name change in disability certificate and UDID card in Jharkhand
On this subject the rules are unusually clear. Carry them with you.

We build these pages by reading the law rather than other websites. On most subjects that produces silence. Here it produced a list, and the list is the most useful thing we can give you.

Provisions of the Rights of Persons with Disabilities Act 2016 and the rules made under it that matter to a name correction
What the law saysWhat it means at the counter
"The certificate of disability issued under this section shall be valid across the country."A Jharkhand certificate works everywhere in India. Nobody in another State may treat it as a local paper, and a family that has moved does not need to start again
Application is made in Form IV to a medical authority or other notified competent authorityA named form. Ask for it by that name. On most subjects this website has to tell readers that no form number exists, and here one does
The application is accompanied by "proof of residence", "two recent passport size photographs" and an "aadhaar number or aadhaar enrollment number, if any"Note the words if any. Unlike several schemes on this website, the rules here do not make an Aadhaar an absolute condition of applying
"No other proof of residence shall be demanded from the applicant who has aadhaar or aadhaar enrollment number."A protection, and a quotable one. If you have given an Aadhaar and somebody asks for three more residence papers, this sentence is your answer
"The medical authority shall issue the certificate of disability within a month from the date of receipt of the application."One month, written in the rules. Which is why the date of receipt matters so much, and why you should never hand in an application without getting it acknowledged
Where an applicant is found ineligible, the medical authority "shall convey the reasons to him in writing under Form VIII"A refusal must be explained, on paper, on a named form. A verbal no is not what the rules provide for. Ask for the written reasons
A permanent certificate is issued where there are no chances of variation of the disability over time, and otherwise a certificate indicating the period of validitySo two different documents exist and they behave differently. Know which one you are holding before you plan anything around it
Certificates issued under the earlier 1995 Act "shall continue to be valid" after the new Act beganDo not surrender an old certificate. An older document with an older spelling is still a document, and it is evidence of what was recorded and when
The State Government is to ensure that the certificate is granted on an online platform, and there is a right of appeal against a certifying authority's decisionSo there is a system behind the counter and a route above it. Both of those are dealt with further down this page
How to use this table, practically. Print it, or write the three sentences that matter on one sheet: valid across the country, one month, and no other proof of residence where an Aadhaar is given. Take that sheet with you and keep it in your file, not to argue with anybody, but because a person who can name the rule is treated differently from a person who cannot. That is simply how offices work, and it is free.

The Disability Certificate and the UDID Card Are Two Documents From One System

People telephone us saying the name on their card is wrong, and the first thing we have to establish is which of the two papers they are holding. They come out of the same system but they are not the same document.

How the certificate of disability and the UDID card differ and how they are connected
The certificate of disabilityThe UDID card
A document issued by a certifying authority after an assessment, under the Act and the rules made under itAn identity card issued through the Unique Disability ID system, described officially as giving a Unique Disability Identity Card to each person with disabilities
Its legal standing comes from the Act itself, which says it "shall be valid across the country"Its purpose is described as "transparency, efficiency and ease of delivering the government benefits" along with uniformity
The thing a college, an employer, a bank or an office is usually actually asking to seeThe thing people carry, and the thing whose name is most often noticed to be wrong first, because it is looked at more often
Issued in a form the rules prescribe, with either permanent validity or a stated period of validityProduced by the same portal, which is described as an "end-to-end integrated system for Issuance of Universal ID & Disability Certificates"
Correct this, and the card should follow, because the certificate is the document with legal standing behind itCorrecting only the card while the certificate still carries the old spelling leaves the real problem in place
Where the name on the certificate itself is wrong, the request goes to the authority that issued itThe official description of the system lists "Online renewal and update of information by Person with Disabilities" among its own functions
The sentence that changes the tone of the whole conversation. Updating your information is described as a function of the system, not as an exception somebody has to be persuaded to make. On several other subjects this website has had to report the opposite, that the name is simply absent from what a person may edit. Here the system's own description says renewal and update of information by the person with the disability is part of what it does. Say that plainly and politely, and put your request in writing.
One honest limit before you go further. The official portal pages for this system would not open properly for us to read, so we print no screen by screen method, no correction form number for the card, no fee and no timeline for an online update. What we print is what the Act, the rules and a Government institute's published description of the system actually say. If any website gives you a confident click by click method, check where they say it came from, because we could not verify one.

Never Apply Again Under the Corrected Name to Fix a UDID Spelling

This is the single most expensive mistake made on this subject, and it is made with the best intentions. A family finds the spelling wrong, the correction route looks slow, and somebody suggests simply applying afresh with the right name. It does not work, and it makes things worse.

Why it does not work. The Unique Disability ID system is described in its own published objectives as providing "Online availability of data of Person with Disabilities across country through a centralized web application" and "Non-duplication of PwDs data". Non-duplication is not a side effect, it is a design goal. A second application in a slightly different name is exactly the thing a national database built for non-duplication is watching for.
1

You end up with two records, not one correct one

The first record does not disappear because a second one was made. You now have a problem that has to be untangled rather than a spelling that had to be corrected, and untangling is always the harder job.

2

The old record is still the one with history

Benefits taken, concessions used, admissions and employment records that quoted the earlier number. A fresh record has none of that behind it, and the old one does not stop existing.

3

A second assessment is a burden nobody needs

An application is followed by an assessment by the certifying authority. Nobody should be put through that again because of a spelling, and a correction request does not require it.

4

It can look like something it is not

Two records for one person in a database built to prevent exactly that will be questioned, and the questions will land on the family. An honest correction never puts you in that position.

5

The correction route exists and is named

The system's own description includes renewal and update of information by the person with the disability. Use the route that exists rather than the one that seems quicker.

6

If a second record already exists, say so first

Do not hide it and hope. Disclose it in writing, with both papers attached, and ask for the position to be regularised. A problem you declared yourself is a very different thing from one that was discovered.

What to do instead, in one line. Write to the authority that issued the certificate, attach the certificate and the Aadhaar, state the correct spelling, ask for the particulars to be corrected in the existing record and for the card to be reissued accordingly, and get the application acknowledged. One application, one record, one file. That is slower on the first day and very much faster over a year.

A Name Correction Is Not a Fresh Disability Assessment

A great many families live with a wrong spelling for years because of one fear: that touching the certificate will reopen everything and put the person through the whole assessment again. It is worth separating the two things carefully.

The difference between assessing a disability and correcting a particular on the certificate
What an assessment isWhat a correction of particulars is
The Act says the Central Government "shall notify guidelines for the purpose of assessing the extent of specified disability in a person"A spelling is not an extent. A name is a particular recorded on a document, and correcting it does not touch the finding the certificate records
The certifying authority "shall assess the disability of the concerned person in accordance with relevant guidelines" and then issue the certificateYour request is not asking anybody to assess anything. It is asking for the person's name on an existing finding to be written correctly
Where the disability may vary over time, the certificate carries a period of validity, and that is the designThat periodic review is a separate thing with its own reason. Do not confuse it with a correction, and do not let anybody else confuse the two either
A permanent certificate is issued where there are no chances of variation of the disability over timeA permanent certificate with a wrong spelling is still a permanent certificate. Correcting the spelling is not a reason to convert it into something else
How to keep the two apart in your applicationSay it in words: this is an application for correction of particulars in an existing certificate and is not a request for fresh assessment. Put that sentence in. It is short and it does real work
What we have not verified, said plainlyWe did not find a published rule that expressly states a correction of particulars never triggers a re-assessment. So we are giving you the distinction the Act itself draws and the words to use, not a guarantee about how a particular office will handle it
Why this fear is worth confronting rather than living with. A wrong spelling on a disability certificate does not stay a small problem. It surfaces at an admission, at a recruitment verification, at a concession counter, at a bank, and each time it has to be explained by the person least able to be kept waiting. The correction is a single piece of work. The explaining is a lifetime of small ones. Our name mismatch at document verification page describes what that looks like when it is left until the day it is checked.

Written Reasons, the One Month and the Statutory Right of Appeal

On most subjects on this website, a refusal is the end of the practical road and we have to say so honestly. Here it is not, because an appeal is written into the Act itself.

The provision to know. The Act says: "Any person aggrieved with decision of the certifying authority, may appeal against such decision, within such time and in such manner as may be prescribed by the State Government." A statutory right of appeal, with the time and the manner left to the State Government to prescribe. Very few of the records described on this website carry anything like it.
1

Get the application acknowledged on the day

The rules give the medical authority one month from the date of receipt. That clock cannot start, and cannot be proved to have started, without an acknowledgement in your hand.

2

Ask for the reasons in writing if you are refused

The rules provide that where an applicant is found ineligible, the authority shall convey the reasons in writing under Form VIII. A verbal refusal is not what the rules describe.

3

Read the reasons before you react

Sometimes a refusal turns on a missing paper rather than the merits, and that is fixed by sending the paper. The written reasons tell you which kind of no you have received.

4

Find out the State's prescribed time and manner

The Act leaves both to the State Government. Ask the issuing office in writing what the prescribed appeal period and route are, and note the date you asked, because that request is itself useful paper.

5

Appeal on paper, briefly

What you asked for, what was decided, why you say it was wrong, and what you want done. Attach the written reasons and the acknowledgement. Short beats long in front of any authority.

6

Keep the whole file together

Application, acknowledgement, reasons, appeal, and every certificate you have ever held including an older one. The file is what speaks for you when officers change.

What we could not verify about Jharkhand, and will not invent. The Act leaves the appeal time and manner to the State Government, and leaves the designation of certifying authorities to the appropriate Government. We have not verified Jharkhand's own notification of certifying authorities, its prescribed appeal period, the appellate authority or any district level arrangement. So this page names no officer, prints no address and states no appeal deadline. Ask the issuing office in writing and get the answer on paper, because that answer is the thing you need and nobody on the internet can give it to you reliably.

How to Correct a Name in a Disability Certificate and UDID Card in Jharkhand

1

Work out which document is wrong

The certificate, the card, or both. The certificate is the document with legal standing, so if it carries the wrong spelling that is where the work belongs, and the card should follow it.

2

Put it beside the Aadhaar and the family papers

The school certificate, the birth certificate, the bank passbook. Decide which spelling the whole household is going to carry before you ask anybody to change anything.

3

Do not apply afresh, whatever anybody suggests

The system is built for non-duplication. A second application makes two records instead of one corrected one, and that is a worse position than the one you started in.

4

Write to the authority that issued the certificate

Not to a general office. The issuing authority is the one that can correct its own record, and its name is usually printed on the certificate you already hold.

5

Say expressly that this is a correction of particulars

And that it is not a request for a fresh assessment. One sentence, near the top. It answers a question the office would otherwise ask, and it settles a fear the family has.

6

Attach the right papers and nothing extra

The existing certificate, the Aadhaar, the document that shows the correct spelling. If you give an Aadhaar, no other proof of residence may be demanded, and that is the rules' own wording.

7

Get it acknowledged, with the date

Because the rules run a one month timeline from the date of receipt. Never hand in an application without taking a receipt away with you, however friendly the counter is.

8

Keep the old certificate, do not surrender it

Certificates issued under the earlier 1995 Act continue to be valid, and any certificate you have held is evidence of what was recorded and when. Photocopy everything you hand over.

9

Ask for the card to be reissued after the certificate is corrected

In the same application or a following one. Renewal and update of information is named as a function of the system, so this is a request within its own design.

10

If refused, ask for the reasons in writing

The rules provide for written reasons on Form VIII where an applicant is found ineligible. Read them before deciding what to do next.

11

Use the appeal the Act gives you

Ask the office in writing for the State's prescribed appeal time and manner, then appeal briefly, with the reasons and the acknowledgement attached.

12

Then settle the records that read this one

Aadhaar, school and board records, bank account. One agreed spelling across everything is the real goal, and our name spelling correction page explains how to choose it.

Where most readers of this page will end up. One written application to the issuing authority, acknowledged, and a corrected record. That is genuinely the whole job in most cases, and it is not a job that needs an agent. Where we earn a fee is in the households where four or five documents disagree and somebody has to decide which single spelling everything is moving to, and in drafting so that an office is not given a reason to send the file back.

Central Gazette Process for a Name From Jharkhand, and Where It Fits a Disability Certificate

Read this before the rest of this section. A wrongly spelt disability certificate does not need a gazette notice. It needs a written application to the authority that issued it, asking for the particulars to be corrected. This process matters to you in one situation only: where the person is genuinely changing their own name, and the certificate, the card, the Aadhaar and the school or board records all have to follow afterwards. It is printed in full so you can see what a real file involves, and so you recognise at once when a gazette is being sold to a family whose certificate merely carries a spelling somebody wrote down by ear. The answer there is no, and the correct answer costs a sheet of paper.

No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.

The Eight Steps of a Name File Sent From Jharkhand

1

Settle the final name first

Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.

2

Pick the category, then its proforma

A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.

3

Type the proforma in duplicate

Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.

4

Fill the employment field if it is a change of name

The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.

5

Publish and keep the original paper

One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.

6

Pay only through BharatKosh

The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.

7

Check every date, then post it yourself

Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.

8

Read the notice, then update in the right order

Check every word the day it publishes, download the e-Gazette copy, and then work through the Aadhaar first, then the certificate with the issuing authority, then the card, then the school, board and bank records. Keep every earlier certificate rather than surrendering it.

The Five Papers in the File, and What Is Checked in Each

The five papers of a Central Gazette name file from Jharkhand and what the Department checks in each
The paperWhat is checked in it, clause by clause
1. The proforma, in duplicateThat it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number
2. The affidavit or undertakingThe same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default
3. The original newspaperThe original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year
4. Photographs, identity proof and any category paperTwo self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error
5. The CD, its certificate and the BharatKosh receiptMS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt
The proforma is where files are lost, and there are twelve to choose from. A wrongly typed spelling filed on the change of name form declares henceforth be known as, claiming a change you never made. The Government charge already paid is not refunded, because BharatKosh payments are non-refundable. You then have six months to make good a shortcoming the Department points out, after which a fresh application with all new documents is required.
What to select on BharatKosh, field by field. Pay to the Department of Publication under the Ministry of Housing and Urban Affairs, and pick the Ministry, the Pay and Accounts Office and the Drawing and Disbursing Officer by their names in the dropdown, never by a code copied off a website. The codes printed for this office on different websites do not agree with each other, and money paid into the wrong head cannot be traced to your file. Read the amount on the portal on the day you pay, because it is revised each financial year.
Why a gazette is the wrong route for a misspelt certificate. A gazette notice is a published statement that a person is changing their name. A family whose certificate merely carries a spelling written down by ear is not changing anybody's name, and filing a change of name declaring henceforth be known as claims something that did not happen. The route here is an application for correction of particulars to the issuing authority, which is quicker, cheaper and truthful. Our name correction against name change page sets out the difference, and our public notice for a wrongly written name page covers the category that does exist for genuine errors.

The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.

What a Disability Certificate and UDID Name Correction Costs in Jharkhand

Cost and time for correcting a name in a certificate of disability or a UDID card in Jharkhand
ItemCostTime
Deciding which spelling the household will carryNothing, and it has to be done first. The certificate, the Aadhaar and the school or birth record on one table, read letter by letter. Choose once and carry that everywhereHalf an hour
A written application for correction of particularsPaper and postage. Addressed to the authority that issued the certificate, stating that it is a correction and not a request for fresh assessmentThe rules give one month from the date of receipt for issuing a certificate
Getting the application acknowledgedNothing, and never skip it. The one month runs from the date of receipt, and an acknowledgement is the only way to show when that wasSame day, at the counter
Asking for written reasons after a refusalNothing. The rules provide that reasons are conveyed in writing on Form VIII where an applicant is found ineligible, so this is asking for what the rules already requireAsk in writing and note the date
An appeal against the certifying authority's decisionA right given by the Act itself. The time and manner are left to the State Government to prescribe, and we have not verified Jharkhand's, so we print no period and no feeAsk the issuing office in writing for the prescribed time and manner
The correction or reissue of the card itselfWe print no fee, because we verified none. The portal pages would not open for us, and the system's own description names renewal and update of information as one of its functionsNot stated in any source we could read
Correcting the Aadhaar, where that is the record that is wrongAadhaar charges a fee per attempt with no refund and limits how often a name may be updated. Get the request right the first timeSet by the enrolment centre
Government printing charge, only in a genuine change of a person's own nameThe guidelines print Rs 1,100 for an adult change of name and Rs 1,400 for a public notice, for FY 2016 to 2017, each with the caveat that the charge varies each financial year and from case to case. What BharatKosh shows on the payment screen the day you pay is the figure that countsPart IV publishes weekly, so allow several weeks from posting
Our chargeQuoted on your papers before you pay anything, and confirmed in writing. We print no service price on any page of this site
Do not trust a figure printed on any website, including this one, on the day you pay. The Department's own guidelines say the printing charge varies each financial year and from case to case, so the figures above are what the published guidelines show for 2016 to 2017 and are given so you know the order of the amount, not the amount. What BharatKosh shows when you reach the payment screen is the one that counts. Read it there and pay that.
Look at how much of this table is free, and say so to anybody who quotes you a price. The application, the acknowledgement, the written reasons and the appeal are all things the Act and the rules already provide. For a straightforward misspelling there is very often nothing at all for a family to pay, and we would rather tell you that than take a fee for writing one letter. Where we are genuinely useful is a household whose documents have drifted apart over twenty years, and our name change cost page sets out the line items across every record.

Myths About Disability Certificate and UDID Name Corrections

Common beliefs about correcting a name in a certificate of disability or UDID card and what is actually correct
What families are toldWhat is actually so
Just apply again with the correct spelling, it is fasterThe most damaging advice on this subject. The system's published objectives include "Non-duplication of PwDs data". A second application makes two records, not one corrected one, and the first does not go away
Touching the certificate means the whole assessment starts againAssessment is about the extent of a specified disability, under guidelines notified for that purpose. A spelling is a particular, not an extent. Say in your application that you are asking for a correction of particulars and not a fresh assessment
A certificate from Jharkhand will not be accepted in another StateThe Act says "The certificate of disability issued under this section shall be valid across the country." That is the whole answer, and it is worth carrying in writing
They can ask for as many residence proofs as they likeThe rules say "No other proof of residence shall be demanded from the applicant who has aadhaar or aadhaar enrollment number." Quote it politely if you are asked for a pile of papers
There is no time limit, so it will take as long as it takesThe rules say "The medical authority shall issue the certificate of disability within a month from the date of receipt of the application." Which is exactly why the acknowledgement matters so much
If they say no, that is the end of itIt is not. A refusal is to be conveyed in writing on Form VIII, and the Act gives a right of appeal to "Any person aggrieved with decision of the certifying authority"
An Aadhaar is compulsory before you can even applyThe rules ask for an "aadhaar number or aadhaar enrollment number, if any". Those last two words matter, and they are unlike several schemes described elsewhere on this website
The old certificate from before the new Act is useless now, throw it awayKeep it. Certificates issued under the earlier 1995 Act "shall continue to be valid", and any certificate you have held is evidence of what was recorded and when
Fix the card and the certificate will sort itself outThe other way round. The certificate is the document with legal standing behind it, so correct that first and ask for the card to be reissued to match it
A gazette notification is needed to correct the spellingNo. This is a correction of particulars by the issuing authority. A gazette declares that somebody is changing their name, which is not what has happened when a clerk wrote a spelling by ear
The question that ends most of these conversations. Ask: which document actually carries the wrong spelling, and who issued that document? Once you can name the issuing authority, this stops being a mystery about a portal and becomes an ordinary written request to a named office, backed by an Act, rules, a named form, a one month timeline and a right of appeal. Very few subjects on this website give you that much to stand on.

Key Terms in a Disability Certificate and UDID Name Correction

Key terms used when dealing with a wrong name in a certificate of disability or UDID card
TermWhat it means in practice
Certificate of disabilityThe document issued after assessment by a certifying authority. The one with legal standing, and the one to correct first
UDID cardThe Unique Disability Identity Card issued through the national system. The document people carry, and usually the first place a wrong spelling is noticed
Certifying authorityPersons designated by the appropriate Government, "having requisite qualifications and experience", who are competent to issue the certificate. The office your request goes to
Medical authorityThe authority named in the rules to receive the application and issue the certificate within a month from the date of receipt
Form IVThe form in which an application for a certificate of disability is made under the rules. Ask for it by name
Form VIIIThe form on which reasons are conveyed in writing where an applicant is found ineligible. A refusal is meant to be explained on paper
Permanent certificateIssued where there are no chances of variation of the disability over time. It does not carry an expiry, and a spelling correction does not change that
Period of validityStated on a certificate where the disability may vary over time. A separate thing from a correction, with its own reason
AssessmentDeciding the extent of a specified disability under notified guidelines. What a certificate records, and not what a spelling correction touches
Correction of particularsAsking for a name, or another recorded detail, to be written correctly in an existing record. The words to use in your application
Non-duplicationA stated objective of the national system. The reason a second application under a new spelling creates a problem rather than solving one
AppealThe right given by the Act to a person aggrieved by a certifying authority's decision, in the time and manner prescribed by the State Government

Disability Certificate and UDID Name Help Across Jharkhand

The office that matters here is the one that issued the certificate, and its name is usually printed on the certificate you already hold. What we do sits before that visit, and it is mostly drafting and deciding. We read the certificate against the Aadhaar and the rest of the family's papers, help you settle on the one spelling everything is going to carry, draft the application for correction of particulars in the words that make clear it is not a request for fresh assessment, tell you what to attach and what nobody may demand from you, and tell you what to do if the answer comes back as no. Everything on this subject can be done on scans over WhatsApp, which matters when travelling to an office is itself the hardest part of the job. If the person concerned cannot easily attend, tell us that in your first message, because it changes how we draft.

Send the certificate and the Aadhaar

Send the certificate of disability as it stands, the UDID card if one has been issued, the Aadhaar, and whichever other papers carry the name: a school or board certificate, a birth certificate, a bank passbook. Send an older certificate too if the family still has one, because a document issued years ago is evidence of what was recorded and when, and we would rather see it than guess. Tell us in one line what you actually want: the spelling corrected, the card reissued to match, or both. Our first reply tells you which document is the one to correct, which office issued it, what the application should say so that it is understood as a correction of particulars rather than a request for fresh assessment, and what nobody is entitled to demand from you. For a straightforward misspelling we will usually tell you that this is one letter and that you do not need to pay anybody, including us. Details on the contact page.

Disability Certificate and UDID Name Change: Frequently Asked Questions

How do I correct the name on my disability certificate?+
Write to the authority that issued it and ask for a correction of particulars. Attach the certificate, the Aadhaar and the document that shows the correct spelling. Say expressly that this is an application for correction of particulars and not a request for fresh assessment, because that one sentence answers the question the office would otherwise ask. Get the application acknowledged with the date, because the rules give the medical authority one month from the date of receipt. Then ask for the card to be reissued to match the corrected certificate.
Can I simply apply again with the correct spelling?+
No, and this is the most important warning on the page. The system's own published objectives include "Non-duplication of PwDs data" and "Online availability of data of Person with Disabilities across country through a centralized web application". A second application does not replace the first; it creates a second record in a database built to catch exactly that. You would be turning a spelling problem into a duplication problem, and the first record, with all the history attached to it, does not go away.
Will correcting the name mean the assessment is done all over again?+
The two things are different, and the Act keeps them apart. Assessment is about the extent of a specified disability, carried out under guidelines notified for that purpose. A name is a particular recorded on a document, not an extent. Put the words correction of particulars, not a fresh assessment in your application. We did not find a rule stating in so many words that a correction never triggers a re-assessment, so we are giving you the distinction the Act itself draws and the wording to use, rather than a guarantee about one office.
Is my Jharkhand certificate valid in another State?+
Yes, and the Act says so. It provides that "The certificate of disability issued under this section shall be valid across the country." A family that has moved for work, study or treatment does not need to start again elsewhere, and nobody is entitled to treat it as a local document. Carry that sentence in writing if you expect an argument.
They are asking me for several residence proofs. Is that right?+
The rules say the application is accompanied by proof of residence, two recent passport size photographs and an "aadhaar number or aadhaar enrollment number, if any", and then add plainly: "No other proof of residence shall be demanded from the applicant who has aadhaar or aadhaar enrollment number." Quote that politely rather than arguing. A person who can name the rule is usually treated differently from one who cannot.
How long should it take?+
The rules say "The medical authority shall issue the certificate of disability within a month from the date of receipt of the application." That is why we press so hard on one small thing: never hand in an application without taking a dated acknowledgement away with you. The month runs from the date of receipt, and without a receipt you cannot show when that was.
What if my application is refused?+
Ask for the reasons in writing. The rules provide that where an applicant is found ineligible, the medical authority shall convey the reasons in writing on Form VIII. Read them, because a refusal often turns on a missing paper rather than the merits. Then, if you need to, use the appeal the Act itself gives: "Any person aggrieved with decision of the certifying authority, may appeal against such decision, within such time and in such manner as may be prescribed by the State Government."
What is the appeal period in Jharkhand?+
We have not verified it and we will not print a guess. The Act leaves the time and the manner of appeal to the State Government to prescribe, and leaves the designation of certifying authorities to the appropriate Government. So ask the issuing office in writing what the prescribed appeal period and route are, and note the date you asked. That written request is itself useful paper, and the answer is the thing you actually need. No website can give it to you reliably, including this one.
Do I correct the certificate or the UDID card first?+
The certificate first. It is the document with legal standing behind it, issued by a certifying authority under the Act. The card is issued through the national system, which describes itself as an "end-to-end integrated system for Issuance of Universal ID & Disability Certificates". Correcting the card while the certificate still carries the old spelling leaves the real problem where it was.
Should I throw away my old certificate from before the new law?+
Keep it, and do not surrender it. Certificates issued under the earlier 1995 Act "shall continue to be valid" after the new Act commenced. Beyond that, any certificate a person has held is evidence of what was recorded and when, which is exactly what you may need to show if anybody later asks why two spellings exist. Photocopy everything you hand over to any office, every time.
Is there a gazette route for this?+
Not for a misspelling, no. A gazette notice declares that a person is changing their name. A family whose certificate carries a spelling somebody wrote down by ear is not changing anybody's name, and filing a change of name would claim something that did not happen. The correct route is an application for correction of particulars to the issuing authority. Our name correction against name change page explains the difference, and our public notice for a wrongly written name page covers the category that does exist for genuine errors.
How much of this can you actually do for us?+
We read the certificate against the Aadhaar and the rest of the family's papers, help settle the one spelling everything will carry, draft the application for correction of particulars in wording that makes clear it is not a request for fresh assessment, tell you what to attach and what nobody may demand, and tell you what to do if the answer is no. For a straightforward misspelling we will usually tell you this is one letter and that you need not pay anybody, us included. We are a documentation consultancy and not advocates, not a medical authority, not a certifying authority and not the Department of Empowerment of Persons with Disabilities. We give no medical opinion of any kind.

Who wrote and checked this page

Written and maintained by the Jharkhandnamechange.com documentation desk, led by founder Vipin Chauhan, B.Tech LL.B, with 5+ years filing with the Department of Publication. The positions quoted on this page come from three sources and we keep them separate. From the Rights of Persons with Disabilities Act 2016: that the Central Government "shall notify guidelines for the purpose of assessing the extent of specified disability in a person"; that the appropriate Government "shall designate persons, having requisite qualifications and experience, as certifying authorities, who shall be competent to issue the certificate of disability"; that on an application the certifying authority "shall assess the disability of the concerned person in accordance with relevant guidelines notified under section 56, and shall, after such assessment, as the case may be, issue a certificate of disability to such person"; that "The certificate of disability issued under this section shall be valid across the country"; and that "Any person aggrieved with decision of the certifying authority, may appeal against such decision, within such time and in such manner as may be prescribed by the State Government." From the Rights of Persons with Disabilities Rules 2017: that an application is made in Form IV to a medical authority or other notified competent authority; that it is accompanied by "proof of residence", "two recent passport size photographs" and an "aadhaar number or aadhaar enrollment number, if any"; that "No other proof of residence shall be demanded from the applicant who has aadhaar or aadhaar enrollment number"; that "The medical authority shall issue the certificate of disability within a month from the date of receipt of the application"; that a permanent certificate is issued where there are no chances of variation of the disability over time and otherwise a certificate indicating a period of validity; that where an applicant is found ineligible the medical authority "shall convey the reasons to him in writing under Form VIII"; that the State Government is to ensure the certificate is granted on an online platform; and that certificates issued under the Persons with Disabilities (Equal Opportunities, Protection of Rights and Full Participation) Act 1995 "shall continue to be valid" after the new Act commenced. From the published description of the Unique Disability ID project by a Government of India institute: that its objectives include "creating a National Database for PwDs, and to issue a Unique Disability Identity Card to each person with disabilities" and to "encourage transparency, efficiency and ease of delivering the government benefits" along with uniformity; that it is an "end-to-end integrated system for Issuance of Universal ID & Disability Certificates for Person with Disabilities"; and that it provides "Online availability of data of Person with Disabilities across country through a centralized web application", "Non-duplication of PwDs data" and "Online renewal and update of information by Person with Disabilities". Four limits are declared on the page itself. First, the official portal pages for this system would not open properly for us to read, so we print no screen by screen method, no correction form number for the card, no fee and no timeline for an online update. Second, we have not verified Jharkhand's own notification of certifying authorities, its prescribed appeal period and manner, its appellate authority or any district level arrangement, so the page names no officer, prints no address and states no appeal deadline, and tells the reader to ask the issuing office in writing instead. Third, we did not find a rule stating in so many words that a correction of particulars never triggers a re-assessment, so the page gives the distinction the Act itself draws and the wording to use, and says openly that it is not a guarantee. Fourth, the Act and rules quoted are the Central Act and the Central rules, and States make their own rules under the same Act, so a reader should check the Jharkhand position with the issuing office. The framing of the subject as a case where the law is already on the reader's side, the warning against applying afresh because the system is built for non-duplication, and the separation of a correction of particulars from an assessment, are our own working practice and are described as such. We give no medical opinion of any kind and express no view on anybody's assessment. The gazette category positions and the printing charges shown are from the Department of Publication's own published guidelines for FY 2016 to 2017, with the guidelines' own caveat that they vary each financial year and from case to case. We are a documentation consultancy and not advocates, not the Department of Publication, not the Department of Empowerment of Persons with Disabilities, not a medical authority, not a certifying authority and not a hospital. Who we are is on the about page.
