ECHS Card Name Correction in Jharkhand: Ex-Servicemen, Dependants and the Record Office

Thousands of ex-servicemen and their families in Jharkhand depend on the Ex-Servicemen Contributory Health Scheme (ECHS). ECHS's own FAQ answers the exact question: "What should I do if the details on the Card viz. name, date of birth, entitlement, etc. are wrong?" The answer is to log in, "click for change of data", make the changes, pay online, and the application "will go to the respective Record office for verification. After verification the card will be printed." For a spelling slip, that is not a Gazette notice. We publish Gazette notices, and we say this first.

1Office that verifies: your Record Office
1Temporary slip to keep treatment going meanwhile
0Gazette notices needed for a slip
Quick answer

Five points, in the order you will need them. First, log in to the ECHS online card portal and choose change of data. Second, correct the name exactly as your service records show it, because the Record Office checks it. Third, pay online where asked; StaffNews reproduces the ECHS instruction of Rs 177 per card for a change to a read-only field. Fourth, download the temporary slip and get it countersigned at your parent polyclinic, so treatment continues. Fifth, a dependant's name is corrected the same way. We publish Gazette notices, and we say plainly that most ECHS name corrections need none.

What ECHS's Own FAQ Says About a Wrong Name on the Card

ECHS card name correction in Jharkhand, with the ECHS smart card, discharge book and Aadhaar of an ex-serviceman
The Record Office checks the change before a new card is printed.

ECHS's FAQs of 10 April 2023 answer question 128 directly. The whole process is online, with verification by the Record Office.

ECHS FAQ answer on a wrong name, step by step
StageWhat ECHS's FAQ says
Log inAt the ECHS card portal "using login credentials and click for change of data"
Edit"An auto populated application will open and the beneficiary can carry-out required changes"
Pay"Make necessary payment online"
VerifyThe application "will go to the respective Record office for verification"
Keep treatment goingA temporary slip, "countersigned from the OIC of parent polyclinic"
The Record Office decides. StaffNews reproduces the ECHS instruction that the beneficiary "is responsible to fill correct data", and that it is "checked by the respective Record Office for correctness". So the name you enter should match your service records, not just your Aadhaar.

Which ECHS Card Name Mismatch You Have

ECHS name problems usually fall into a few kinds. This sorting is ours, built on the Record Office check.

Kinds of ECHS card name mismatch
What does not matchWhat it usually is
Card against service recordsA slip when the card data was entered
Service records against AadhaarAn old slip in the service record, or in Aadhaar
Wife's name on the cardA spelling slip, or a name written differently
Child's name on the cardA slip against the child's own papers
Widow's card after the ex-serviceman's deathHer name as in the family pension record
If the service record itself is wrong, start there. The Record Office checks the card against it. How a service record is corrected is a matter for your Record Office, and we did not verify that procedure. For the pension side, see our pension records page.

How to Fix Each ECHS Card Name Mismatch

The ECHS route is the same online change of data for every case. What to fix first is our practical advice.

Fixing each kind of ECHS card name mismatch
MismatchWhat usually fixes it
Card slip, service record rightChange of data online, matching the service record
Aadhaar slipCorrect Aadhaar with UIDAI. See Aadhaar name change
Service record wrongAsk your Record Office how to correct it first
Dependant's slipChange of data for that dependant, with their own papers
Genuine change of nameA Central Gazette notice, then Aadhaar, then the Record Office and ECHS
Keep the pension record in step. The same name should appear on the ECHS card, the pension payment order and the bank account. See our Jeevan Pramaan page, where a mismatch can hold up the life certificate.

ECHS Read-Only Fields, Dependants' Cards and the Rs 177 Fee

StaffNews reproduces an ECHS instruction on changing data after the 64 KB smart card is printed. It explains that some fields cost money to change and some do not, and that changing the primary card affects the family's cards.

ECHS rules on changing card data, as reproduced by StaffNews
PointWhat the instruction says
First stepBlock the card, selecting the reason "change in data"
Read-only fields, marked yellowChanges need "payment INR 177/- per card"
Read-write fields, marked greenNo payment
Primary beneficiary edits a read-only field"all his/her dependent cards will also get blocked and payment for all the cards will be required"
After verificationSent to the vendor for printing and dispatch to the parent polyclinic
Count the cards before you edit. If you change your own name in a read-only field, every dependant's card may need paying for too. Check the portal for which fields are read-only for you, and confirm the current fee there, since we read a reproduction of the instruction.

Does an ECHS Card Name Correction Need a Gazette Notice?

No, for a slip. ECHS's own FAQ sends a wrong name to change of data and Record Office verification. A dependant's slip goes the same way. A Gazette notice is for a genuine change of name.

When a Gazette notice is relevant to an ECHS card
SituationWhat usually applies
Card slipChange of data. No Gazette
Dependant's slipChange of data. No Gazette
Aadhaar slipUIDAI correction. No Gazette
Genuine new nameA Central Gazette notice is the clean route
Our commercial interest, stated plainly. We publish Gazette notices. For an ECHS name slip, you do not need one, and a veteran should not be sold one. Where you have genuinely changed your name, it goes in the Gazette of India, Part IV, since the Jharkhand state press stopped personal notices in July 2025, as reported.

How to Correct a Name on an ECHS Card, Step by Step

1

Check every name on the ECHS card

Yours and each dependant's.

2

Compare with your service records

Discharge book, PPO.

3

Fix Aadhaar first if it is wrong

With UIDAI.

4

Ask the Record Office if the service record is wrong

Before the card.

5

Log in to the ECHS card portal

With your credentials.

6

Block the card for change in data

Select the reason.

7

Edit the name

Exactly as in the records.

8

Pay where asked

Rs 177 per card for read-only fields, as reproduced.

9

Download the temporary slip

Get it countersigned at the polyclinic.

10

Collect the new card

From your parent polyclinic.

The one rule that holds the whole job together. Match the service record. The Record Office checks the card against it.

Central Gazette Process for a Name From Jharkhand, and Where It Fits an ECHS Card

Read this before the rest of this section, because an ECHS name slip never needs it. A card slip or a dependant's slip goes through change of data; an Aadhaar slip goes to UIDAI. No Gazette notice is needed for those. It is printed in full so you recognise at once when a notice is being sold to a veteran who does not need one.

No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.

The Eight Steps of a Name File Sent From Jharkhand

1

Settle the final name first

Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.

2

Pick the category, then its proforma

A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.

3

Type the proforma in duplicate

Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.

4

Fill the employment field if it is a change of name

The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.

5

Publish and keep the original paper

One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.

6

Pay only through BharatKosh

The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.

7

Check every date, then post it yourself

Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.

8

Read the notice, then update in the right order

Check every word the day it publishes and download the e-Gazette copy. Then update Aadhaar, then ask your Record Office, then apply for change of data on the ECHS portal.

The Five Papers in the File, and What Is Checked in Each

The five papers of a Central Gazette name file from Jharkhand and what the Department checks in each
The paperWhat is checked in it, clause by clause
1. The proforma, in duplicateThat it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number
2. The affidavit or undertakingThe same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default
3. The original newspaperThe original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year
4. Photographs, identity proof and any category paperTwo self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error
5. The CD, its certificate and the BharatKosh receiptMS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt
The proforma is where files are lost, and there are twelve to choose from. A wrongly typed spelling filed on the change of name form declares henceforth be known as, claiming a change you never made. The Government charge already paid is not refunded, because BharatKosh payments are non-refundable. You then have six months to make good a shortcoming the Department points out, after which a fresh application with all new documents is required.
What to select on BharatKosh, field by field. Pay to the Department of Publication under the Ministry of Housing and Urban Affairs, and pick the Ministry, the Pay and Accounts Office and the Drawing and Disbursing Officer by their names in the dropdown, never by a code copied off a website. The codes printed for this office on different websites do not agree with each other, and money paid into the wrong head cannot be traced to your file. Read the amount on the portal on the day you pay, because it is revised each financial year.
What a Gazette notice will not do for an ECHS card. It will not change the card by itself; you still apply for change of data, and the Record Office still verifies. It links old and new names, and nothing more. Since we publish these notices, the honest version is this: for an ECHS name slip, you need none. Our name correction against name change page explains the categories.

The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.

Who wrote and checked this page

Written and maintained by the Jharkhandnamechange.com documentation desk, led by founder Vipin Chauhan, B.Tech LL.B, with 5+ years filing with the Department of Publication. The correction route on this page is quoted from ECHS's FAQs of 10 April 2023, question 128 on a wrong name and question 112 on dependants. The block-card step, read-only fields, the Rs 177 fee and dependants' cards are from the ECHS instruction on change of data as reproduced by StaffNews. Three limits are declared openly on the page itself. First, ECHS's own copy of that instruction did not load for us, so the fee should be confirmed on the portal. Second, we did not verify how a service record is corrected. Third, which record to fix first is our advice. We publish Gazette notices, and this page says plainly that an ECHS name slip needs none. We are a documentation consultancy and not advocates, not ECHS, not a Record Office, and not the Department of Publication. Who we are is on the about page.
