CGHS Card Name Correction in Jharkhand: Beneficiary Name, Family Members and Aadhaar

Central Government employees and pensioners in Ranchi use the Central Government Health Scheme (CGHS). A wrong spelling on the CGHS card, or a family member's name that does not match Aadhaar, can hold up Aadhaar linking and treatment paperwork. CGHS's own guidance, as reproduced by GConnect, says that for any data error beneficiaries should "contact CMO i/c of CGHS Wellness Centre for correction of Data". The card guidelines of 28 June 2024 ask for "Aadhaar Card/PAN card or any other valid document as per RBI guidelines". So the fix is at your wellness centre, with your documents. For a spelling slip, that is not a Gazette notice. We publish Gazette notices, and we say this first.

1Officer to correct CGHS data: the CMO in charge of your wellness centre

Quick answer

Five points, in the order you will need them. First, check every name on the card: yours and each family member's. Second, compare each with Aadhaar, since CGHS links Aadhaar numbers to beneficiary IDs. Third, for a data error, go to the CMO in charge of your wellness centre with the correct document. Fourth, for a pensioner, the name should also match the PPO. Fifth, tell CGHS at once when a family member's status changes, as the guidelines require. We publish Gazette notices, and we say plainly that most CGHS name corrections need none.

How a CGHS Card Carries Your Name and Your Family's Names

CGHS card name correction in Jharkhand, with the CGHS e-card, Aadhaar and pension payment order
Each family member on the card should match their own Aadhaar.

The Ministry of Health's office memorandum F. No. S.11012/1/2024-EHS sets out how CGHS cards are issued. Serving employees "apply for a new CGHS card online" and the department forwards it; pensioners apply to the Additional Director, CGHS.

Documents and records behind a CGHS card, per the 2024 guidelines
RecordWhat the guidelines say
Identity"Aadhaar Card/PAN card or any other valid document as per RBI guidelines"
PhotographsIndividual photographs of each beneficiary
Pensioner's record"Self-attested PPO/ Provisional PPO or Last pay certificate"
Subscription"Copy of Bharat Kosh Challan for CGHS subscription paid", for pensioners
The card itselfAn electronic card on the myCGHS and DigiLocker apps
Aadhaar is linked to the beneficiary ID. GConnect's reproduction of CGHS guidance shows an "Update Aadhar No." option after logging in with the beneficiary ID and an OTP. If the system says "details for selected beneficiary not found", the advice is to check the details or contact the CMO in charge. A name slip can be one cause.

Which CGHS Card Name Mismatch You Have

CGHS name problems usually fall into a few kinds. This sorting is ours, built on the documents the guidelines ask for.

Kinds of CGHS card name mismatch
What does not matchWhat it usually is
Cardholder's name against AadhaarOften a data entry slip
Wife recorded by her maiden nameA change by marriage
Child's name against the child's AadhaarOften a short name or a spelling slip
Pensioner's name against the PPOA slip in one of the two records
Aadhaar itself is wrongAn Aadhaar correction first
Fix Aadhaar first if Aadhaar is wrong. CGHS links Aadhaar to each beneficiary. If Aadhaar has the slip, correct it with UIDAI, then ask CGHS to correct the card. See our Aadhaar name change page.

How to Fix Each CGHS Card Name Mismatch

The CGHS guidance we read sends data corrections to the CMO in charge of your wellness centre. Which document to carry is our practical advice.

Fixing each kind of CGHS card name mismatch
MismatchWhat usually fixes it
Data entry slip on the cardAadhaar showing the right name, to the CMO in charge
Wife's maiden nameHer marriage certificate with her Aadhaar. See name change for married women
Child's nameThe child's Aadhaar or birth certificate
Pensioner's name differs from PPOFix whichever is wrong. See pension records
Genuine change of nameA Central Gazette notice, then Aadhaar, then CGHS and the service or pension record
Serving employees: your department is part of the chain. The guidelines route a serving employee's application through the department. If your service record has the slip, fix it there too. See our government employees page.

CGHS Family Members and Pensioners: Keeping Every Name Right

A CGHS card covers dependent family members, and each one carries a name. The guidelines say beneficiaries must "inform CGHS immediately" of dependency changes, and that failure may lead to withdrawal of the facility.

Family and pensioner situations on a CGHS card
SituationWhat to do
New wife to be addedAdd her with Aadhaar and the marriage certificate
Daughter marriedInform CGHS; dependency may change
Retiring from serviceApply for a pensioner card with the PPO and subscription challan
Pensioner has diedThe family pensioner's own card and PPO; names must match
Dependency rules are not covered here. Who counts as a dependant depends on CGHS rules on income and relationship, which we did not verify. Ask your wellness centre before adding or removing anyone. For the ration card side of adding a wife, see our adding wife to ration card page.

Does a CGHS Card Name Correction Need a Gazette Notice?

Usually not. A data entry slip is corrected by the CMO in charge against Aadhaar. A wife's maiden name is linked with a marriage certificate. A child's name follows Aadhaar or the birth certificate. A Gazette notice is for a genuine change of name.

When a Gazette notice is relevant to a CGHS card
SituationWhat usually applies
Data entry slipCMO correction with Aadhaar. No Gazette
Wife's maiden nameMarriage certificate. No Gazette
Child's spellingAadhaar or birth certificate. No Gazette
Genuine new nameA Central Gazette notice is the clean route
Our commercial interest, stated plainly. We publish Gazette notices. For most CGHS name corrections, you do not need one, and a retired employee should not be sold one. Where you have genuinely changed your name, it goes in the Gazette of India, Part IV, since the Jharkhand state press stopped personal notices in July 2025, as reported.

How to Correct a Name on a CGHS Card, Step by Step

1

Open your CGHS e-card

On myCGHS or DigiLocker.

2

Check every name on it

Yours and each family member's.

3

Compare each with Aadhaar

Letter by letter.

4

Fix Aadhaar first if it is wrong

With UIDAI.

5

Pensioners: compare with the PPO

Fix whichever is wrong.

6

Gather the proof

Aadhaar, marriage or birth certificate.

7

Visit your wellness centre

Ask for the CMO in charge.

8

Request correction of data

With self-attested copies.

9

Link Aadhaar in the CGHS portal

Login with beneficiary ID.

10

Check the corrected e-card

The new name should show.

The one rule that holds the whole job together. Aadhaar right first, then the CMO in charge. CGHS links each beneficiary to Aadhaar.

Central Gazette Process for a Name From Jharkhand, and Where It Fits a CGHS Card

Read this before the rest of this section, because most CGHS name corrections never need it. A data slip is corrected by the CMO in charge; a wife's maiden name uses a marriage certificate; a child's name follows Aadhaar. No Gazette notice is needed for those. It is printed in full so you recognise at once when a notice is being sold to a pensioner who does not need one.

No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.

The Eight Steps of a Name File Sent From Jharkhand

1

Settle the final name first

Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.

2

Pick the category, then its proforma

A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.

3

Type the proforma in duplicate

Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.

4

Fill the employment field if it is a change of name

The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.

5

Publish and keep the original paper

One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.

6

Pay only through BharatKosh

The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.

7

Check every date, then post it yourself

Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.

8

Read the notice, then update in the right order

Check every word the day it publishes and download the e-Gazette copy. Then update Aadhaar and PAN, then your service or pension record, then ask the CGHS wellness centre to correct the card.

The Five Papers in the File, and What Is Checked in Each

The five papers of a Central Gazette name file from Jharkhand and what the Department checks in each
The paperWhat is checked in it, clause by clause
1. The proforma, in duplicateThat it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number
2. The affidavit or undertakingThe same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default
3. The original newspaperThe original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year
4. Photographs, identity proof and any category paperTwo self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error
5. The CD, its certificate and the BharatKosh receiptMS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt
The proforma is where files are lost, and there are twelve to choose from. A wrongly typed spelling filed on the change of name form declares henceforth be known as, claiming a change you never made. The Government charge already paid is not refunded, because BharatKosh payments are non-refundable. You then have six months to make good a shortcoming the Department points out, after which a fresh application with all new documents is required.
What to select on BharatKosh, field by field. Pay to the Department of Publication under the Ministry of Housing and Urban Affairs, and pick the Ministry, the Pay and Accounts Office and the Drawing and Disbursing Officer by their names in the dropdown, never by a code copied off a website. The codes printed for this office on different websites do not agree with each other, and money paid into the wrong head cannot be traced to your file. Read the amount on the portal on the day you pay, because it is revised each financial year.
What a Gazette notice will not do for a CGHS card. It will not change the card by itself; the wellness centre still corrects it. It links old and new names, and nothing more. Since we publish these notices, the honest version is this: for most CGHS name corrections, you need none. Our name correction against name change page explains the categories.

The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.

What a CGHS Card Name Correction Costs

The 2024 guidelines say "No fee shall be levied, in case the beneficiary opts for renewal/reissue of card without a fresh printed plastic card." The other costs are the documents, and usually not a Gazette notice.
