IEPF Unclaimed Shares and Dividend Refund Name Mismatch in Jharkhand: IEPF-5 Claim Guide
Many families in Jharkhand find old share certificates in a parent's almirah, in a name like "S. R. Sharma", while the claimant's PAN and Aadhaar read the full name. When dividends go unclaimed, the shares move to the Investor Education and Protection Fund (IEPF), and the claim back is made on Form IEPF-5. SEBI's own FAQ for share registrars gives this exact case: a name on the certificate as "S. R. Shah or Sushil R Shah" against a demat account in the full name "would be considered as minor mismatch". That is fixed with Aadhaar or PAN, not a Gazette notice. We publish Gazette notices, and we say this first.
Five points, in the order you will need them. First, initials or a short name on the share certificate against your full name is a minor mismatch; SEBI's FAQ says the registrar takes a self-attested copy of a document such as Aadhaar, PAN or passport to explain it. Second, a woman's name changed by marriage is proved with a marriage certificate, or a passport showing the husband's name. Third, any other genuine change of name needs a Gazette notice and a document in the old name, per the same FAQ. Fourth, your demat account, bank account, PAN and Aadhaar must all carry the same name before you file IEPF-5. Fifth, shares of a parent who has died go through transmission first. We publish Gazette notices, and we say plainly that most IEPF name problems need none.
How an IEPF-5 Claim Checks Your Name: Company, Registrar and IEPF
You first get an entitlement letter from the company or its registrar, then file IEPF-5 online, then send papers to the company's Nodal Officer. Sun Pharma's published claim procedure, as one example, lists what goes to the company. Every paper carries a name.
| Paper | Whose name, and against what |
|---|---|
| Printout of the filed IEPF-5 and acknowledgement | The name you typed on the form |
| "Original Indemnity Bond", auto-generated after filing | Signed by the claimant |
| Self-attested Aadhaar and PAN | Your name as the Government holds it |
| "Original Cancelled Cheque leaf containing name of the claimant(s)" | The bank's name for you |
| Client master list of the demat account | Your name as the depository holds it |
| Share certificates or statement, and the entitlement letter | The name the company recorded long ago |
Which IEPF Name Mismatch You Have: Minor or Major
SEBI's FAQ on investor service requests splits name problems into two kinds. Which one yours is decides everything that follows.
| What does not match | What it usually is |
|---|---|
| Initials on the certificate, full name on PAN | Minor mismatch, per SEBI's own example |
| A spelling slip, or the middle name missing | Usually a minor mismatch |
| A woman's maiden name on the certificate | A change by marriage |
| A completely different name | A genuine change of name |
| The holder has died | Transmission to the heir, then the claim |
How to Fix Each Name Mismatch Before Filing IEPF-5
SEBI's FAQ sets the papers for each kind. For a minor mismatch, "the RTA shall additionally obtain self-attested copy of any one of the documents mentioned in Table 3, clarifying the difference in names".
| Mismatch | What SEBI's FAQ asks for |
|---|---|
| Minor mismatch | One of: Aadhaar, passport, driving licence, a Government or bank photo ID, PAN with photograph, marriage certificate, divorce decree |
| Change by marriage | "Marriage Certificate or copy of valid Passport showing husband's name or publication of name change in official gazette, and any document evidencing the old name" |
| Change for any other reason | "Publication of name change in official gazette, and any document evidencing the old name" |
| Your own KYC names differ | Fix PAN, Aadhaar, demat and bank to one name first. See PAN and Aadhaar mismatch |
IEPF Claim for a Deceased Parent's Shares: Transmission and Names
An heir cannot claim in the dead holder's name. The shares are first transmitted to the heir, then claimed. SEBI's FAQ covers transmission documents separately, and it does not have a specific answer on name mismatch in transmission. So the names on the death certificate and the heir's papers must be set right first.
| Paper | What usually fixes a wrong name |
|---|---|
| Death certificate | Registrar correction. See death certificate correction |
| Deceased's initials on the certificate | A minor mismatch, explained with the deceased's Aadhaar or PAN |
| Heir's own name across PAN, demat and bank | Bring them to one name first |
| Joint holder has died | A "certified true copy of Death certificate", per Sun Pharma's procedure |
Does an IEPF Claim Name Mismatch Need a Gazette Notice?
For initials, a short name or a spelling slip, no. SEBI calls that a minor mismatch and accepts Aadhaar, PAN or similar. For a married woman, usually no: a marriage certificate or a passport with the husband's name works. But for a genuine change of name for any other reason, SEBI's FAQ asks for the Gazette. This is one of the few places where the rule names it.
| Situation | What SEBI's FAQ asks for |
|---|---|
| Initials or short name | Aadhaar, PAN or similar. No Gazette |
| Spelling slip | Usually a minor mismatch. No Gazette |
| Change by marriage | Marriage certificate or passport. Gazette optional |
| Change for any other reason | Gazette notice required, with a document in the old name |

Central Gazette Process for a Name From Jharkhand, and Where It Fits an IEPF Claim
No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name File Sent From Jharkhand
Settle the final name first
Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.
Type the proforma in duplicate
Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.
Fill the employment field if it is a change of name
The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then update in the right order
Check every word the day it publishes and download the e-Gazette copy. Then update PAN and Aadhaar, then the demat account and bank, and give the notice with an old-name document to the registrar.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | The same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
