Name Spelling Correction Affidavit in Jharkhand: Two Formats and Where They Work
A spelling correction affidavit is the cheapest paper in this whole subject, and the one most often worded wrongly. Its job is to say, on oath, that a name was written wrongly, not that you changed it. One wrong phrase, "hitherto known as", turns a clerk's typing error into a change of name you never made. The Notaries Act lets a notary "administer oath to, or take affidavit from, any person", so the paper itself is easy to get. What is harder is knowing where it works. The Reserve Bank's KYC rule names a Gazette notification or a State Government marriage certificate, not an affidavit. SEBI tells share registrars they "shall not insist on Affidavits" for KYC updates. And a board corrects against its own register, not against your oath. This page gives you two formats and tells you honestly where each one will and will not carry you.
Five points, in the order you will need them. First, decide whether it is a correction or a change. A spelling correction affidavit is for a name that was written wrongly. If you chose a new spelling, that is a change of name, and this affidavit is the wrong paper. Second, never use the words "hitherto known as" or "henceforth". Those words declare a change of name. A correction affidavit says the name "has been wrongly written as" in a named document, and what the correct name is. Third, pick the right format. Use Format One for a typing error in one document, and Format Two where two spellings appear across several documents and you need to say both belong to one person. Fourth, get it sworn properly. The Notaries Act lets a notary "administer oath to, or take affidavit from, any person"; type it on non-judicial stamp paper, Rs 100 as our safe default, sign it in front of the notary and keep the original. Fifth, know where it works. It usually helps with an employer, a school or an office that asks for an explanation. It does not replace what the Reserve Bank names for banks, the Gazette notification or a State Government marriage certificate. SEBI says registrars "shall not insist on Affidavits" for KYC updates. And a board such as JAC corrects against the school's admission register. Where the error sits in the original register itself, the Department of Publication's public notice for a wrongly written name is the stronger paper.
Spelling Correction Affidavit or Name Change Affidavit: Which One You Need
Two affidavits look alike and mean opposite things. A correction affidavit says: my name was always this, and a record got it wrong. A change affidavit says: I was known by one name, and from now on I will be known by another. Swear the wrong one and you have put a false statement on oath, and made the next office's job harder, not easier.
| Your situation | Which paper |
|---|---|
| A clerk typed Sunita as Sunitha on one certificate | Correction affidavit, Format One |
| Your name is Kumar on some records and Kumaar on others, and you always used Kumar | Correction affidavit, Format Two |
| A Hindi name written two ways in English | Format Two. See Hindi to English spelling |
| You chose a new spelling, for numerology or any reason | Not a correction. A change of name. See name change for numerology |
| The wrong spelling is in the school's own admission register | The affidavit alone rarely settles it. See JAC register wrong spelling |
| Your father's name is spelt differently | A separate affidavit. See father's name affidavit |
Name Spelling Correction Affidavit Format One: A Typing Error in One Document
Use this where one document carries a wrong spelling and your other records are right. This is our own format, drafted for Jharkhand; if the office you are sending it to prescribes its own wording, use theirs. Replace everything in brackets.

The Clauses an Office Reads For in a Spelling Correction Affidavit
An officer reading your affidavit has two minutes and three questions: who is this, what is wrong, and what is right? Every clause should answer one of those. Anything that does not answer one is noise.
| Clause | Why an office reads for it |
|---|---|
| Full correct name, parentage and address | Identifies you. Match the address to your Aadhaar |
| The wrong spelling, exactly as printed | So the officer can match it to the record in front of him |
| The document, its number and issuing office | Without it, the affidavit explains nothing specific |
| "One and the same person" | The line that links the two spellings |
| "I have not changed my name" | Marks it as a correction, not a change |
| The anchor document | Says which spelling is correct and why |
| Verification with place and date | The closing statement that the contents are true. Without it, it is not an affidavit |
Where a Spelling Correction Affidavit Works, and Where It Does Not
An affidavit is your own statement on oath. It explains; it does not prove the record wrong. Some offices accept that explanation. Others go by their own rules, which name other papers. Here is what we can tell you from the rules we have read.
| The office | What its rule says |
|---|---|
| Banks | The Reserve Bank's KYC rule names an officially valid document with a Gazette notification or a State Government marriage certificate. It does not name an affidavit. See bank KYC |
| Share registrars | SEBI says the registrar "shall not insist on Affidavits or Attestation / Notarization or indemnity" for KYC updates. SBI's own shareholders' format is an "Affidavit for Minor Correction in Name" |
| JAC and school records | Corrected against the school's admission register. The affidavit supports; the register decides. See JAC certificate correction |
| Employers and recruitment panels | Often helpful to explain a gap, while you correct the record itself |
| The Gazette public notice for a wrongly written name | The Department's file includes an attested copy of the connected document carrying the error. See public notice for a wrongly written name |
| Every other office | Ask what it accepts before you swear anything. One phone call saves a stamp paper |
How to Make a Name Spelling Correction Affidavit in Jharkhand, Step by Step
Decide it is a correction, not a change
The name was written wrongly; you did not choose a new one.
Find your anchor document
Usually the Class 10 certificate, the oldest reliable record.
List every document and its spelling
With document numbers. This list becomes the affidavit.
Ask the office what it accepts
One call tells you whether an affidavit helps there.
Choose Format One or Format Two
One document wrong, or two spellings across several.
Strike out "hitherto known as" and "henceforth"
They declare a change of name.
Type it on non-judicial stamp paper
Rs 100 as our safe default, in your own name.
Swear it before a notary
Sign in front of the notary, and check the seal and registration number.
Hand a copy to the office with the correction request
Keep the original, and show it if asked.
Get the record itself corrected
The affidavit explains; the corrected record is what lasts.
Central Gazette Process for a Name From Jharkhand, and Where a Spelling Affidavit Stops and a Notice Begins
No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name File Sent From Jharkhand
Settle the final name first
Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.
Type the proforma in duplicate
Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.
Fill the employment field if it is a change of name
The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then update in the right order
Check every word the day it publishes, download the e-Gazette copy, and then update the Aadhaar first, because the system leverages it for unique identification, and let the PAN, the bank account and any investment records follow after it in that order. Take the notice to each office with the correction request, and keep your affidavit with it.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | The same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
