Mother Name Change Affidavit Format in Jharkhand
There is no single mother name affidavit, and that is why so many are wasted. A mother name question splits three ways, and each way has its own wording. Her name may be wrongly printed on a record, which is a correction. Her name may have genuinely changed at marriage, from Kumari or Devi to a husband surname, which is a real change and not a misprint. Or two proper forms of her name may both be in use, which is a one and the same person declaration. Swear the change of name wording over a printing mistake and you have sworn to a change that never happened. This page shows you which one you are in, and the wording for each.
Decide which of three things is actually true before anybody drafts anything, because the wording changes completely and the wrong wording is worse than no affidavit at all. One, her name is wrongly printed. A record says Sunita where her documents say Sunita Devi, or the spelling is simply wrong. Nothing about her name changed. The correct wording says the name is wrongly mentioned or printed as one thing while the correct name is another, and that the two refer to one and the same person. The State Bank of India publishes an affidavit of exactly this shape, with a field reading "my/our name/surname/father's name/mother's name ... is wrongly mentioned /printed as" and the operative line that the two are "the one and the same person". Two, her name genuinely changed. She was Sunita Kumari before marriage and became Sunita Verma after it. That is a real change and it should be sworn as one. Passport Seva publishes a proforma in this shape: "I, (Old Name) (S/o, D/o, or W/o) (Name), resident of (full address) do hereby declare that I have changed my name from (Old Name) to (new/ Correct Name)", with the confirmation that both names are the "same and one identical person". Three, two proper forms are both in use across different records, and you simply need an office to accept that they are the same woman. That is a one and the same person declaration and nothing more. Then one more question that is particular to a mother name and that nobody asks: who swears it, she or the child? Get that wrong and a perfectly worded affidavit still fails.
Three Situations Behind a Mother Name Affidavit, and Only One Is Yours
Put your papers side by side and answer one question: did her name actually change, or was it written down wrongly? Everything follows from that.
| What is actually true | Which affidavit it needs |
|---|---|
| Her name is wrongly printed on a record, and nothing about her name ever changed | A wrongly written or correction affidavit. It states the name as printed, the correct name, and that the two are one and the same person. It must not say she changed her name |
| Her name genuinely changed at marriage, from Kumari or Devi to a husband surname | A change of name affidavit, because a real change happened. This is where the change wording belongs and nowhere else |
| Two proper forms of her name are both in use across her records, and both were properly issued | A one and the same person declaration. Nothing is being changed or corrected, you are asking an office to accept that both refer to her |
| Her name is blank on the record | Not an affidavit question at first. Get a copy of the register entry, because a blank can mean several things and needs the record read before anything is drafted |
| The record carries a completely different woman name | Also not a template case. That is an error in substance and it needs the papers looked at, not a downloaded format |
Mother Name Affidavit Format: The Operative Wording for Each of the Three
An affidavit is only as good as its operative clause, the one sentence that says what is actually being declared. Everything above it is your name, your father or husband name, your address and your age. Everything below it is the verification. The clause in the middle is the whole document, and it is the clause that changes between the three situations.
Two published forms are worth reading before you draft anything, because both are real and both are free to look at. They are not Jharkhand forms and they are not made for a mother name, but they show you the shape that Government offices and banks already accept.
What the Two Published Forms Actually Say
| The published form | The wording it uses |
|---|---|
| Passport Seva name change proforma, for a genuine change of name | It reads "I, (Old Name) (S/o, D/o, or W/o) (Name), resident of (full address) do hereby declare that I have changed my name from (Old Name) to (new/ Correct Name)", and goes on to confirm that both names refer to the "same and one identical person". Note the two halves: the change is declared, and then the identity is tied together |
| State Bank of India Name Correction Affidavit, for a wrongly printed name | Its field reads "my/our name/surname/father's name/mother's name ......... is wrongly mentioned /printed as", and its operative line is that the name as on one document and the name as on the supporting documents "is the one and the same person". Note what is absent: there is no sentence saying anybody changed a name |
Situation One: Her Name Is Wrongly Printed
Use this when nothing about her name ever changed and a clerk simply wrote it wrong, or wrote a short form of it. The clause should do three things and nothing more: quote the name exactly as printed on the record, state the correct name, and tie the two to one woman.
Situation Two: Her Name Genuinely Changed at Marriage
Use this when she really was Sunita Kumari and really did become Sunita Verma. This is the bucket that exists for mothers and barely exists for fathers, and it is the one people wrongly try to squeeze into a correction affidavit because they think a marriage is somehow not a proper reason.
Situation Three: Two Proper Forms Are Both in Use
Use this when both versions were properly issued and neither is wrong. Sunita Devi on one record, Sunita Kumari Devi on another. You are not correcting anything and you are not changing anything. You are asking an office to accept that both are her.
Who Swears a Mother Name Affidavit, the Mother or the Child
This is the part that is particular to a mother name and it is the part that is got wrong most often. A well drafted affidavit sworn by the wrong person is still refused. Work it out from whose record is being fixed, not from who is available to go to the notary.
| Whose record carries the problem | Who should depose |
|---|---|
| Your record, for example your school certificate or your passport application, where her name is printed wrongly as a parent entry | You depose, because it is your document and you are stating a fact about your own record. Her own papers go in as the supporting documents. Many offices also like her affidavit alongside yours, and there is no harm in both |
| Her own record, for example her Aadhaar, her PAN or her bank account | She deposes, always. You cannot swear a change or a correction to another adult own document. If she cannot travel, the notary can be brought to her, or a local notary used where she lives |
| She has genuinely changed her name at marriage and her own records are being brought in line | She deposes. This is her name and her change, and on her own documents nobody else can declare it for her |
| She has died, and her name is wrong on your record | You depose, stating the date of her death and annexing the death certificate, and produce whatever of her documents the family holds. Do not draft anything in her voice. An affidavit is sworn by a living person |
| You are still a minor and the record is your school record | A parent or guardian deposes for you. The Passport Seva proforma itself carries a minor version in which a parent or guardian declares on the child behalf, which is the same principle |
How to Get a Mother Name Affidavit Made in Jharkhand, Step by Step
Lay out her own documents first
Her Aadhaar, PAN, voter card, bank passbook and school certificate, side by side. Whatever form of her name most of her own papers carry today is the target. Never pick a form from your record and work backwards to hers.
Decide which of the three you are in
Wrongly printed, genuinely changed, or two proper forms in use. Write the answer down in one sentence before anybody drafts. If you cannot write that sentence, you are not ready for a notary.
Ask the office for its own format
Before paying for a draft, ask the school, bank or office in writing whether it has a prescribed format. Banks often do, and the State Bank of India publishes one. Using the office own form removes every argument about wording.
Settle who deposes
Her record, she deposes. Your record, you depose. She has died, you depose with the death certificate. Fix this before the stamp paper is bought, because the deponent name is printed on it.
Buy the stamp paper in the deponent name
Rs 100 non-judicial stamp paper is our safe default across Jharkhand. It must be bought in the name of the person who will sign, not in the name of whoever goes to the vendor.
Quote the wrong entry exactly as printed
Letter for letter, spacing and all, with the document named and its number given. An affidavit that paraphrases the wrong entry is useless at a counter that is comparing it against the certificate.
Attest before a notary, and annex the proofs
A notary is enough for an ordinary correction or identity affidavit. Annex her identity documents, and the death certificate where she has died. Take two originals, because offices keep one.
File it with the application, not on its own
An affidavit never fixes a record by itself. It goes in with the written application to the school, board, bank or Registrar, and you keep a copy of the application carrying the office received stamp.
Where her own name is being changed across her own records, the route is the one on our mother name change and correction page. Where the problem is the entry on a birth record, start at name change in birth certificate. The general affidavit rules across Jharkhand are on our name change affidavit page, and the father side of the same question, which splits two ways rather than three, is on the father name change affidavit format page.
Central Gazette Process for a Name From Jharkhand, and When an Affidavit Is Not Enough
No district in Jharkhand has a gazette counter, so the file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name Change File Sent From Jharkhand
Settle the final name first
Write it out letter by letter, with the spacing wanted, and then use exactly that in every paper. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
For a woman taking a husband surname after marriage this is the married woman route, not an ordinary change of name. Twelve categories exist and only one is yours, and picking the wrong one is how files come back.
Type the proforma in duplicate
Two originals, signed in the old name, with two witnesses giving signature, full name, address and mobile number on both copies. The old name has to be quoted exactly as it is printed on each certificate named.
Get the affidavit attested at the right level
A notary for the ordinary change of name, and a First Class Magistrate of the concerned area where the different surname guidelines apply. This is the step that catches people out.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then download it
Check both names letter by letter the day it publishes, note the page and issue number, and download the e-Gazette copy of that Part IV issue.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as her documents carry it, surname included; the new full name exactly as settled; the father or husband name; the full address with pin code; the signature in the old name; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | Signed by her, because it is her name; the same names and address as the proforma, word for word; attested at the level the category asks for; on Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both of her names carried; the same spelling as the proforma; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; identity and address proof matching the proforma address; both witnesses proofs; and whatever the category itself demands, such as a marriage certificate or a no objection certificate where the maiden surname requirement applies |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the wording on the proforma format page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
What a Mother Name Affidavit Costs in Jharkhand
| Item | Cost | Time |
|---|---|---|
| The affidavit itself, any of the three types | Rs 100 non-judicial stamp paper as our safe default, plus the notary charge. The wording does not change the price, so pay for the right wording rather than the cheapest draft | The same day |
| A second original | Another stamp paper and attestation. Worth it, because offices keep the copy you hand in and you are left with nothing | The same visit |
| Where the office has its own format | Usually nothing extra. Banks like the State Bank of India publish theirs, and using it removes every argument about wording | Ask in writing, allow a few days for the reply |
| School or board correction that follows | The institution own charge, plus Rs 10 for an RTI application where an admission register page has to be obtained that way. We do not print school or board charges we have not verified | Set by the institution. We do not print a timeline we have not verified |
| Birth register correction that follows | The Registrar office own charge. A correction is made in the margin and the original entry is not altered | Set by the office |
| Government printing charge, only where she is genuinely changing her own name and a gazette is asked for | The adult guidelines print Rs 1,100 for a change of name with effect from 01.04.2016 to 31.03.2017, and Rs 3,500 for an Indian living abroad. Every category guidelines add that the charge varies each financial year and from case to case, so treat these as the order of the amount. What BharatKosh shows on the payment screen the day you pay is the figure that counts | Part IV publishes weekly, so allow several weeks from posting |
| Newspaper, in a gazette case only | Paid to the paper, charged by length | Same week |
| Post or courier to Delhi, in a gazette case only | Sent in her own name from any district, and keep the receipt | A few days each way |
| Our charge | Quoted on your papers before you pay anything, and confirmed in writing. We print no service price on any page of this site | |
For most families reading this page the honest total is a stamp paper and a notary, and nothing else. What makes a mother name case expensive is almost never the affidavit. It is swearing the wrong one, having it refused, and starting again. Our name change cost page sets out the line items in full.
Send the papers and we will draft it
Photograph the record that carries the wrong or different mother name, showing the entry clearly, because the affidavit has to quote it letter for letter. Then send her own documents: Aadhaar, PAN, voter card, bank passbook and her school certificate, because the correct form of her name is whatever most of her own papers carry today. If her name changed at marriage, add the marriage certificate if the family has one. If she has died, add the death certificate. Our first reply tells you which of the three situations you are in, who should depose, and the exact clause for your case. Where the office has its own format, we will tell you to use that instead of paying us for a draft. Details on the contact page.

Myths About a Mother Name Change Affidavit
| What people are told | What is actually so |
|---|---|
| There is one standard mother name affidavit format | There is not. Three different facts need three different clauses, and the wrong clause swears to something that did not happen. Decide the fact first, then draft |
| A child can swear an affidavit changing the mother name | Not on her own documents. Her record, her affidavit. A child may depose about the entry on the child own record, which is a different thing altogether |
| Her marriage name change is a mistake to be corrected | It is not a mistake. A marriage name change is a real change and needs change of name wording. Calling it a printing error in an affidavit is a false statement |
| A printing error needs a change of name affidavit | The opposite. A printing error needs the wrongly mentioned or printed as wording with a one and the same person line, and no sentence saying anyone changed a name |
| A notarised affidavit forces the office to update the record | It does not. An affidavit is a sworn statement that goes with an application. The office corrects its own record against documents it recognises, and the affidavit is only one paper in that file |
| Rs 500 stamp paper is safer than Rs 100 | Paying more does not make an affidavit stronger. Rs 100 non-judicial stamp paper is our safe default. What makes it strong is correct wording, the right deponent and the right supporting papers |
| You need a gazette for a mother name affidavit | Almost never. A gazette is for a person changing her own name. A wrong parent entry on your certificate is a correction at the institution that issued it |
| A downloaded format is good enough | A downloaded format is a starting shape, not a case. It does not know which of the three situations you are in, whose record it is, or what the office asking for it will accept |
Key Terms in a Mother Name Affidavit
| Term | What it means in practice |
|---|---|
| Deponent | The person who swears the affidavit and signs it. On her own record that is your mother. On your record it is you |
| Operative clause | The one sentence that says what is actually being declared. It is the whole document, and it is what changes between the three situations |
| Wrongly mentioned or printed as | The wording for a clerical error. It states the entry as printed and the correct name, and says nothing about anybody changing a name |
| One and the same person | A declaration that two names belong to one woman. Used in a correction and used on its own where two proper forms are both in use |
| Change of name | The wording for a name that genuinely changed, most often at marriage. It declares the change and then ties both names to the same identical person |
| Non-judicial stamp paper | The paper an affidavit is typed on, bought in the deponent name. Rs 100 is our safe default across Jharkhand |
| Notary | The officer who attests the affidavit and enters it in a register. Enough for an ordinary correction or identity affidavit |
| Verification clause | The closing line saying the contents are true to the deponent knowledge and nothing material has been concealed. Signed and dated below the operative clause |
| Annexure | A document attached to the affidavit, such as her Aadhaar or a death certificate. Referred to inside the affidavit by name, not just clipped to it |
| Prescribed format | An office own affidavit form. Where one exists, it beats any draft, and asking for it in writing costs nothing |
| Received stamp | The office acknowledgement on your own copy of the application. Without it, an application never happened |
| Correction | Putting a record right against documents already held. Cheap, quick, publishes nothing |
