SBI and PNB use a Customer Request Form with a "Change my Name to" line • Bank of India leads 15 of Jharkhand's 24 districts • The same Reserve Bank rule binds all three

SBI, Bank of India and PNB Name Change Process in Jharkhand: Forms and Documents

Many people in Jharkhand bank with State Bank of India, Bank of India or Punjab National Bank, and they ask the same question: which form, and which papers? We read the forms the banks publish. State Bank of India's and Punjab National Bank's Customer Request Forms carry the same line: "Change my Name to: ______", with the note "(Relevant document e.g. Govt. Gazette Notification / Marriage Certificate to be attached)". Both also carry a separate section to "Change my signature in above mentioned account", and both say "Your request will be processed within 2 working days." Bank of India, which the Reserve Bank's list shows as lead bank for 15 of Jharkhand's 24 districts, did not let us open its forms page, so we say what we could read and no more. Above all three sits one rule: the Reserve Bank's KYC FAQs accept "an OVD containing the earlier name along with a copy of the Gazette notification, or marriage certificate issued by the State Government".

Quick answer

Five points, in the order you will need them. First, at State Bank of India, use the Customer Request Form. It has a line "Change my Name to", with the note "(Relevant document e.g. Govt. Gazette Notification / Marriage Certificate to be attached)", and asks you to "Attach self-attested copy of document for verification with original". Second, at Punjab National Bank, use form PNB 1250, the Customer Request Form. The copy we read carries the same name change line and the same document note as SBI's. Third, at Bank of India, ask the branch for its form. Its forms page returned an error to us, and the KYC pages we could read describe no name change procedure, so we print none. Fourth, the paper that decides it is the same at all three. The Reserve Bank's KYC FAQs accept an officially valid document in the earlier name along with the Gazette notification or a State Government marriage certificate, and say the customer should submit changed documents "within 30 days" and that the bank "shall acknowledge the receipt". A PAN card is not an officially valid document. Fifth, the signature is a separate section on the SBI and PNB forms, with space for the old and new signature, so tick it only if you want to sign in the new name. The SBI and PNB forms both say requests are "processed within 2 working days", with kits and cheque books taking "between 7-15 working days" to be delivered.

Lead Banks in Jharkhand: Bank of India, SBI and Indian Bank by District

SBI, Bank of India and PNB name change process in Jharkhand with Customer Request Form
Three banks, one Reserve Bank rule, and a form that asks for the same two papers.

The Reserve Bank publishes which bank is the lead bank for each district, meaning the bank that coordinates banking there. For Jharkhand, the list we read shows Bank of India as the convenor of the State Level Bankers' Committee, and as lead bank for most districts. It does not mean other banks are absent; it shows where each bank has taken the lead role.

Lead banks for Jharkhand districts as shown in the Reserve Bank's list
Lead bankJharkhand districts
Bank of India, 15 districtsBokaro, Chatra, Dhanbad, East Singhbhum, Giridih, Gumla, Hazaribagh, Khunti, Koderma, Lohardaga, Ramgarh, Ranchi, Seraikela Kharsawan, Simdega, West Singhbhum
State Bank of India, 7 districtsDeoghar, Garhwa, Jamtara, Latehar, Pakur, Palamu, Sahibganj
Indian Bank, 2 districtsDumka, Godda
Punjab National BankNot a lead bank for any Jharkhand district in the list we read. It still has branches here, and the same rule applies
Why this matters for a name change. If a branch in your district refuses a correct Gazette notice or marriage certificate, the bank is still bound by the same Reserve Bank rule, whichever it is. The lead bank role does not change the rule; it only tells you which bank coordinates in your district. We read this list on a copy published by a tax website, not on the Reserve Bank's own page, so check it there if a district's lead bank matters to you.

SBI Name Change Process: Customer Request Form and Documents

State Bank of India publishes a Customer Request Form that covers most account changes on one sheet. For a name change, the line reads "Change my Name to: ______", and under it: "(Relevant document e.g. Govt. Gazette Notification / Marriage Certificate to be attached)".

The form asks for the bank, branch, date, account number and mobile number at the top. For identity documents it says "Attach self-attested copy of document for verification with original". And on processing: "Your request will be processed within 2 working days. Delivery of kits/cheque book etc. to your address will take between 7-15 working days (depending on delivery location)".

Identity document"Attach self-attested copy of document for verification with original"
Signature change"Change my signature in above mentioned account", from old signature to new signature
Processing time"processed within 2 working days"; kits and cheque books "between 7-15 working days"
SBI's "Affidavit for Minor Correction in Name" is not for your savings account. It turns up in searches, but it is for shareholders correcting a name on share certificates, with Rs 100 special adhesive stamps and attestation by a Notary Public or First Class Magistrate. For an account, the Customer Request Form with the Gazette notice or marriage certificate is what the bank's own form asks for. For shares, see share certificate name change.

PNB Name Change Process: Form PNB 1250 Customer Request Form

Punjab National Bank lists its Customer Request Form as "PNB 1250" on its forms page. The English copy we read carries the same wording as SBI's form for a name change: "Change my Name to: ______", with the note "(Relevant document e.g. Govt. Gazette Notification / Marriage Certificate to be attached)".

It also has the same signature section, numbered 16 on the copy we read: "Change my signature in above mentioned account: From OLD SIGNATURE To NEW SIGNATURE". And the same processing line: "Your request will be processed within 2 working days."

The same words at two banks is useful to you. If an SBI or PNB branch asks for an affidavit, a new Aadhaar or a second notice before changing your name, show the officer the line on the bank's own form: "Relevant document e.g. Govt. Gazette Notification / Marriage Certificate to be attached". It is the bank's own printed instruction. Take our request letter with it if you want the Reserve Bank's sentence in writing too.

Bank of India Name Change Process: What We Could and Could Not Read

Bank of India matters most in Jharkhand, as lead bank for 15 districts including Ranchi, Dhanbad and Bokaro. But its forms page returned an error to us, and the KYC pages we could read describe no name change procedure. So we print no Bank of India form name or wording.

What we could read: Bank of India's KYC page lists accepted documents as the "Passport", "Proof of possession of Aadhar number", the "Voter Identity Card issued by the Election Commission of india", the "Job Card issued by the NREGA duly signed by an officer of State Government", a photograph, and "PAN/form 60" for transactions under the income tax rules. Its KYC updation page, which is written for NRI, PIO and OCI customers, allows submission at the "Home Branch/Any BOI branch" or "By Post/Courier/Email".

The pointWhat we can tell you

Post or email submissionStated on its page for NRI, PIO and OCI customers. We did not find it stated for residents

Do not rely on a Bank of India form copied from a file-sharing site. Old copies of bank forms circulate widely and may be out of date. Ask the branch for the current form, fill it, and attach the Gazette notice or State Government marriage certificate with an officially valid document, as the Reserve Bank's words allow. If the branch refuses, see bank not accepting the Gazette.

The Reserve Bank Rule That Applies to SBI, Bank of India and PNB Alike

Forms differ; the rule above them does not. The Reserve Bank's KYC FAQs say that where your identity document does not carry the new name, you may submit "an OVD containing the earlier name along with a copy of the Gazette notification, or marriage certificate issued by the State Government, as applicable, indicating the change in name."

They also say the customer "shall submit the updated OVD/ other documents to the RE within 30 days of the update/ change/ modification", and "the RE shall acknowledge the receipt of the KYC documents to the customer." RE, regulated entity, means your bank, whichever it is.

The Reserve Bank rules that apply to every bank for a name change
The ruleWhat it means at the counter
Earlier-name OVD plus proofYou do not need a new Aadhaar or passport first
Gazette or State Government marriage certificateEither one, as applicable. After a registered marriage, the certificate may be enough
PAN cardNot an officially valid document on the Reserve Bank's list
Within 30 daysSubmit soon after the change
AcknowledgementAsk for it, and keep the stamped copy
Use both the bank's form and the Reserve Bank's words. The form tells the branch what you want. The Reserve Bank's sentence tells the branch what it must accept. Together they settle most counters in one visit. For the full KYC rule, see bank and insurance KYC.

How to Change Your Name at SBI, Bank of India or PNB, Step by Step

1

Get the proof of the change

The Gazette notification or the State Government marriage certificate.

2

Pick one officially valid document

Passport, driving licence, Aadhaar, Voter's Identity Card, NREGA job card or NPR letter. Not a PAN card.

3

Get your bank's current form at the branch

SBI and PNB: the Customer Request Form. Bank of India: ask for its form.

4

Fill the "Change my Name to" line

The new name exactly as the proof shows it.

5

Tick the signature section only if you want a new signature

Old and new signature in the boxes, signed at the branch if the officer asks.

6

Attach self-attested copies

Of the proof and the identity document, with originals to show.

7

Add our request letter if you want it in writing

It quotes the Reserve Bank's sentence. See the letter format.

8

Hand it in within 30 days of the change

At the branch where the account is held.

9

Get a stamped, dated acknowledgement

The bank "shall acknowledge the receipt", in the Reserve Bank's words.

10

Check the account after 2 working days

The time printed on the SBI and PNB forms. If refused, your stamped copy starts the complaint route.

The one habit that saves the most trouble. Point to the bank's own printed line. At SBI and PNB, the form itself says a Gazette notification or marriage certificate is the document to attach. An officer asking for something else is asking for more than the bank's own form does.

Central Gazette Process for a Name From Jharkhand, and Where SBI, BOI and PNB Fit

Read this before the rest of this section, because after marriage the bank may not need a Gazette notice at all. The Reserve Bank's KYC FAQs accept either "a copy of the Gazette notification, or marriage certificate issued by the State Government" alongside an officially valid document in the old name. So for a surname change after a registered marriage, the certificate may already be enough for the bank. This process earns its cost where the name changed for another reason, or where there is no State Government marriage certificate. It is printed in full so you can judge which situation you are in, and so you recognise at once when a notice is being sold to somebody who does not need one.

No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.

The Eight Steps of a Name File Sent From Jharkhand

1

Settle the final name first

Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.

2

Pick the category, then its proforma

A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.

3

Type the proforma in duplicate

Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.

4

Fill the employment field if it is a change of name

The adult proforma asks for the post of the applicant and the office or company name. A student or a homemaker should still complete it rather than leaving it blank, because a blank field brings the file back.

5

Publish and keep the original paper

One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.

6

Pay only through BharatKosh

The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.

7

Check every date, then post it yourself

Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.

8

Read the notice, then update in the right order

Check every word the day it publishes, download the e-Gazette copy, and then update the Aadhaar first, because the system leverages it for unique identification, and let the PAN, the bank account and any investment records follow after it in that order. Take the notice to the bank with a listed identity document and the bank's Customer Request Form, and keep a stamped copy.

The Five Papers in the File, and What Is Checked in Each

The five papers of a Central Gazette name file from Jharkhand and what the Department checks in each
The paperWhat is checked in it, clause by clause
1. The proforma, in duplicateThat it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number
2. The affidavit or undertakingThe same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default
3. The original newspaperThe original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year
4. Photographs, identity proof and any category paperTwo self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error
5. The CD, its certificate and the BharatKosh receiptMS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt
The proforma is where files are lost, and there are twelve to choose from. A wrongly typed spelling filed on the change of name form declares henceforth be known as, claiming a change you never made. The Government charge already paid is not refunded, because BharatKosh payments are non-refundable. You then have six months to make good a shortcoming the Department points out, after which a fresh application with all new documents is required.
What to select on BharatKosh, field by field. Pay to the Department of Publication under the Ministry of Housing and Urban Affairs, and pick the Ministry, the Pay and Accounts Office and the Drawing and Disbursing Officer by their names in the dropdown, never by a code copied off a website. The codes printed for this office on different websites do not agree with each other, and money paid into the wrong head cannot be traced to your file. Read the amount on the portal on the day you pay, because it is revised each financial year.
What a Gazette notice will not do at the bank on its own. It will not replace an officially valid document; the Reserve Bank's rule needs both together. It will not make a PAN card an officially valid document, and it will not change your specimen signature. Since we publish these notices, the honest version is this: a notice plus a listed document plus a clear letter is what the rule asks for, and if you already have a State Government marriage certificate, you may need no notice at all. Our name correction against name change page explains the categories.

The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.

What a Name Change at SBI, Bank of India or PNB Costs

None of the forms we read prints a charge for a name change. We print none either, because each bank sets its own schedule of charges and it changes. Here is what we can and cannot tell you.

Costs of a name change at SBI, Bank of India or PNB
ItemWhat we can and cannot tell you
The Customer Request FormFree to download from the SBI and PNB websites, or ask at the branch
Any bank charge for the changeNot printed on the forms we read. Ask the branch
New passbook, cheque book or debit cardSet by each bank. We print no figure
An affidavitNot asked for on the SBI and PNB forms for a name change. The SBI affidavit format online is for shareholders
A State Government marriage certificateMay make a Gazette notice unnecessary for the bank
A Central Gazette notice, if you need onePaid only through BharatKosh, revised each financial year, never refunded
What we chargeQuoted on the call. If the bank's form and your existing papers are all you need, we will say so and charge nothing
The mistake that costs the most. Paying for a notarised affidavit because SBI's "Affidavit for Minor Correction in Name" turned up in a search. That format is for shareholders' share certificates. For your account, the bank's own form names the Gazette notification or marriage certificate.

SBI, Bank of India and PNB Name Change: Frequently Asked Questions

How do I change my name in an SBI account?+
Fill SBI's Customer Request Form at the line "Change my Name to", and attach what the form asks for: "Relevant document e.g. Govt. Gazette Notification / Marriage Certificate", with a self-attested copy of an identity document "for verification with original".
Which form does PNB use for a name change?+
Form PNB 1250, the Customer Request Form. The English copy we read has the same "Change my Name to" line and the same note about the Gazette notification or marriage certificate as SBI's form.
How do I change my name in a Bank of India account?+
Ask the branch for its current form. Bank of India's forms page returned an error to us, and the KYC pages we could read describe no name change procedure, so we print none. The Reserve Bank's rule on the documents applies to Bank of India as to every bank.
How long does SBI or PNB take to change the name?+
Both forms say "Your request will be processed within 2 working days." Delivery of kits or a cheque book "will take between 7-15 working days (depending on delivery location)".
Do I need an affidavit for a name change at SBI?+
The SBI Customer Request Form does not ask for one for a name change. SBI's "Affidavit for Minor Correction in Name" found online is for shareholders correcting share certificates, not for savings accounts.
Can I change my name in SBI after marriage without a Gazette?+
The form names "Gazette Notification / Marriage Certificate". The Reserve Bank's FAQs name a "marriage certificate issued by the State Government". With that and an officially valid document, a Gazette notice is usually not needed.
Is my PAN card enough as identity proof at the bank?+
No. A PAN card is not an officially valid document on the Reserve Bank's list. Attach a passport, driving licence, Aadhaar proof, Voter's Identity Card, NREGA job card or NPR letter.
Can I change my signature on the same form?+
Yes, at SBI and PNB. Both forms have a section "Change my signature in above mentioned account" with space for the old and new signature. Tick it only if you want to sign in the new name. See our signature change page.
Which bank is the lead bank in my district?+
In the Reserve Bank's list we read, Bank of India leads 15 Jharkhand districts including Ranchi, Dhanbad and Bokaro, State Bank of India leads 7 including Deoghar and Palamu, and Indian Bank leads Dumka and Godda.
Do the same rules apply at Jharkhand Gramin Bank or a private bank?+
The Reserve Bank's KYC rule applies to every bank it regulates. The form will differ, so ask the branch for its own. Our request letter works with any bank's form.
What if the branch refuses?+
First check the refusal is not right, for example a PAN card offered as the only identity document. If it is wrong, complain to the bank in writing; that is the step the Reserve Bank's Ombudsman scheme needs first. See our bank not accepting the Gazette page.
How much of this can you do for us?+
The checking and the filling. We check your identity document and proof against the Reserve Bank's rule and your bank's form, fill the form and our request letter for you to sign, and tell you what to attach. We cannot submit it for you, and we do not promise what any branch will decide.

The bank forms on this page are quoted from the banks' own published copies. From State Bank of India's Customer Request Form and Punjab National Bank's form PNB 1250, read on the banks' websites: the "Change my Name to" line, the document note, the self-attested copy line, the signature section and the processing times. From SBI's Affidavit for Minor Correction in Name: that it is a shareholders' format. From Bank of India's KYC pages: the documents listed and the modes stated for NRI, PIO and OCI customers. From the Reserve Bank's FAQs on the Master Direction on KYC, dated 9 June 2025: questions 9 and 32. Three limits are declared openly on the page itself rather than buried here. First, Bank of India's forms page returned an error to us, so we print no Bank of India form. Second, we read the Reserve Bank's lead bank list on a copy published by a tax website, not on the Reserve Bank's own page. Third, no bank's charges are printed, because none of the forms we read states one. We are a documentation consultancy and not advocates, not State Bank of India, not Bank of India, not Punjab National Bank, not the Reserve Bank, and not the Department of Publication.
