Name Change in LIC Policy Records in Jharkhand
Every other record on this website is a database entry. A life policy is a contract, and the name on it is the name of a party to that contract, which is why it is altered by an endorsement on the policy bond and not by a form on a portal. That difference matters less than the second one. On a life policy there are two names that have to be right, yours and your nominee's, and almost nobody checks the second. A wrong name on a life policy is usually discovered by the nominee, after a death, at the moment the money is needed and the one person who could have explained it is gone. That is the whole reason to read this page today rather than keep it for later.
Take the policy bond out of the almirah and read two names, not one. First, the life assured. Compare it letter by letter against the identity documents you actually hold today, spacing and initials included. Second, the nominee. LIC's own guidance for policyholders says plainly: "Ensure that the nominees name is correctly incorporated in the policy bond." That sentence exists because this goes wrong often, and because the nominee is the person who will have to prove who they are at the worst possible time. Now the two facts that shape everything else. LIC publishes no form for changing or correcting a name. Its own download list carries forms for claims, maturity, alterations, revival, assignments, loans and nomination, and there is no name change form among them, which means a correction is made by a written request to the servicing branch with the bond and your evidence, and endorsed onto the policy. Nomination, by contrast, does have numbered forms, including Form 3264 for a nomination and Form 3750 for a notice of change of nomination. So there is a printed form for changing who receives the money and none for correcting the name of the person who pays for it. Finally, the rule that decides whether this is easy or impossible: do it while the life assured is alive. Afterwards, the only person who could have sworn to the correction is not there to swear to it.
Two Names on Every LIC Policy, and the One Almost Nobody Checks
People bring us the policy because their own name is spelt wrongly. Nine times out of ten, while we are looking, we find something on the nominee line as well, and that is the one that would have caused the real trouble.
| The name | What goes wrong, and why it matters |
|---|---|
| The life assured, meaning you | A spelling drift, an initial that was never expanded, a surname taken at marriage that the bond still does not carry. This is the one people notice, usually at maturity or when a bank asks. It is a correction, not a change of name, and nothing is published for it |
| The nominee | The name nobody re-reads after the day the proposal was filled. LIC's own policyholder guidance says "Ensure that the nominees name is correctly incorporated in the policy bond", which tells you how common this is. A wrong nominee name is discovered by that nominee, at a claim, without you |
| A wife nominated before marriage records caught up | Very common in this state. She is on the bond in her maiden form while every document she now holds carries her husband surname. Nothing was done wrongly, the papers simply moved on. The position is on our Kumari or Devi to the husband surname page |
| A child nominee entered with a pet name | Raju on the bond, Rajesh on every certificate he now holds. The same trap as a school admission register, and it sits quietly for twenty years. Our short name and full name page explains why that is not a spelling error |
| A nominee who has died, or a nomination never updated | Not a name question at all, and a more serious one. LIC publishes a numbered form for it, Form 3750, notice of change of nomination, and it can be done at any time during the policy |
| The name is right but written in a different order | Same words, different arrangement. Worth raising, but it is a field question rather than a spelling one, and our name order and surname first page sets out how to explain it |
Why There Is No LIC Name Change Form, and What Follows From That
You can check this yourself in about a minute, and it is worth doing before you believe anybody's document list, including ours. Open LIC's own download page and read the forms it publishes.
| What LIC publishes | What that tells you |
|---|---|
| Forms for claims, maturity, survival benefits, alterations, revival, assignments, loans and the salary saving scheme | The routine servicing transactions all have printed forms, because they are routine and the branch handles them the same way every time |
| Numbered nomination forms: 3264 for a nomination, 3265 for a minor nominee, 3237 and 3237(A) for Jeevan Sathi and joint life, 3750 for a notice of change of nomination and 3751 for cancellation, with 3738, 3741 and 3740 for appointing, replacing and revoking an appointee | Who gets the money is treated as a standard, repeatable instruction from the policyholder. Hence the numbers |
| No form for a name change or a name correction of the policyholder or life assured | A name correction is not a standard transaction. It is a written request to the servicing branch, supported by evidence, considered on its merits, and endorsed onto the policy bond if accepted |
| LIC's guidance that for "any servicing matter, contact the Servicing Branch of your policy", and that applications for alterations are made in writing | The servicing branch is the only address that matters. Not the nearest branch, not the branch where you once paid a premium, and not a call centre |
| No published procedure specifically for correcting a policyholder's name | Which is exactly why practice varies between branches, and why any website that gives you a confident, definitive document list is describing one branch's habit as though it were a rule |
Why an LIC Name Mismatch Surfaces at a Claim, and Who It Hurts
This is the part of the page we would ask you to read even if you skip everything else. A life policy is the one record in your file that is used mainly after you are no longer there to explain it, and every other page on this site assumes you will be present to sort out your own paperwork.
| When it comes up | What happens |
|---|---|
| While the life assured is alive, at maturity, a loan against the policy, or a routine update | Awkward but entirely fixable. You are there, you hold the identity documents, you can sign, and the branch can endorse the correction on the bond. This is the window, and it is open right now |
| At a death claim, on the life assured's name | The papers that must agree are the policy bond, the death certificate and the nominee's proof of identity. If the bond carries a form of the name that no other paper carries, the family is left explaining a difference that only the deceased could have explained |
| At a death claim, on the nominee's name | The nominee must show that the person named on the bond is them. Where the bond says Sunita Kumari and her documents say Sunita Verma, she is proving an identity, not collecting a payment, and she is doing it while grieving |
| Where the nominee's bank account is in a different form of the name | A separate link in the same chain, and a common one. The money has to reach an account, and the account name is compared too. Our name change in bank records page covers that end of it |
| Where the nominee has themselves changed name, most often at marriage | Then the chain needs a document connecting the two forms. That is far easier to assemble calmly, years in advance, than at a claim counter |
How to Correct a Name in an LIC Policy in Jharkhand, Step by Step
Read both names on the bond
The life assured and the nominee, against the documents each person actually holds today. Do this for every policy in the house, not only the one that prompted the question.
Name the kind of difference
A spelling drift, an unexpanded initial, a surname taken at marriage, or the same words in a different order. Each has different wording and different evidence, and saying which one it is turns a complaint into a request.
Find the servicing branch
LIC is explicit that for any servicing matter you contact the servicing branch of your policy. It is printed on your documents and it is not necessarily the nearest branch to where you now live.
Ask that branch what evidence it wants
In writing, before assembling anything. There is no published document list, so the branch's answer is the list. This one question prevents the second and third trip.
Decide the final form of the name first
Letter by letter, and the same form you are using on Aadhaar, PAN and the bank. Correcting the policy to a form your other records do not carry simply moves the mismatch.
Submit the written request with the original bond
An endorsement is made on the policy document itself, so the bond has to be produced. Carry the request in two copies and keep one with the received stamp.
Deal with the nominee separately and properly
A nominee name correction is part of the same conversation, but changing the nominee is a different transaction with its own numbered form, 3750. Do not let the two be bundled or confused.
Check the endorsement when the bond comes back
Read the endorsed name letter by letter before you file the bond away again, and tell the family where the bond is kept. A correction nobody can find is not much better than no correction.
The employment records that usually carry the same error are on our EPFO and UAN page, the bank end is on the bank records page, and where a marriage is behind the difference, see name change affidavit after marriage.
Central Gazette Process for a Name From Jharkhand, and Where It Fits an LIC Policy
No district in Jharkhand has a gazette counter, so a genuine file goes from Ranchi, Jamshedpur, Dhanbad, Bokaro, Hazaribagh, Deoghar or anywhere else in the state to the Department of Publication, Civil Lines, Delhi 110054, and the notice publishes in Part IV of the Gazette of India. As reported in July 2025 the Finance Department stopped personal name change notifications from the Jharkhand Government Press with immediate effect, so there is no state alternative to fall back on.
The Eight Steps of a Name File Sent From Jharkhand
Settle the final name first
Letter by letter, with the spacing you want, and then use exactly that everywhere, Aadhaar included. A corrigendum after publication is a second fee and a second wait.
Pick the category, then its proforma
A genuine change of name is change of Name Adult or Major. A wrongly typed spelling is the public notice for a wrongly written name. Twelve categories exist and only one is yours.
Type the proforma in duplicate
Two originals with two witnesses giving signature, full name, address and mobile number on both copies. A change of name is signed in the old name; a public notice is signed in your own name.
Fill the employment field if it is a change of name
The adult proforma asks for the post of the applicant and the office or company name, a field the public notice form does not have. Salaried applicants leave it blank and the file comes back.
Publish and keep the original paper
One of the daily local leading newspapers, carrying both names in full, the father or husband name and the residential address. The whole original paper goes in the file, not a cutting.
Pay only through BharatKosh
The Non Tax Receipt Portal and nowhere else since October 2018. Select the Department of Publication by name, read the amount the portal shows on the day, pay in your own name and print the e-receipt.
Check every date, then post it yourself
Nothing in the file older than one year. Personally or by post or courier only, never through an agent or advocate. Photocopy everything first, because nothing sent in comes back.
Read the notice, then update the records
Check every word the day it publishes, download the e-Gazette copy of that Part IV issue, and then work through Aadhaar first and the policy bond after it.
The Five Papers in the File, and What Is Checked in Each
| The paper | What is checked in it, clause by clause |
|---|---|
| 1. The proforma, in duplicate | That it is the right category format; the existing full name exactly as your documents carry it; the new full name exactly as settled; the father or husband name; the full address with pin code; the post and office or company name where it is a change of name; the signature in the right name for the category; two witnesses complete with signature, full name, address and mobile number |
| 2. The affidavit or undertaking | The same names and address as the proforma, word for word, with wording matching the category. On Rs 100 non-judicial stamp paper as our safe default |
| 3. The original newspaper | The original paper, not a cutting; the paper name and date of publication visible; both names carried; the father or husband name and residential address; dated within the last year |
| 4. Photographs, identity proof and any category paper | Two self attested passport photographs; self attested identity and address proof matching the proforma address; both witnesses proofs; and, in a public notice file, the attested photocopy of the connected document carrying the error |
| 5. The CD, its certificate and the BharatKosh receipt | MS Word, without the witness portion, with the name typed in place of the signature; the certificate that the hard copy and the soft copy are the same; the forwarding letter to the Controller of Publication with a telephone number and email; the e-receipt |
The posting detail is on the sending the file to Delhi page, the change of name wording on the proforma format page and the public notice wording on the public notice proforma page, the charge by category on the fee and undertaking page, the reasons files come back on the rejection reasons page, and the download on the download page.
What an LIC Name Correction Costs in Jharkhand
| Item | Cost | Time |
|---|---|---|
| The written request to the servicing branch | We do not print an LIC charge for this, because LIC publishes no form and no fee for it. Ask the servicing branch what, if anything, it charges. Anyone quoting you a fixed LIC fee has not read the same page we did | We do not print a timeline we have not verified. The branch handling it is the one to ask |
| Changing the nominee, which is a different transaction | A standard servicing transaction on its own numbered form, Form 3750. Again we print no charge, because we have not verified one | Set by the branch |
| Affidavit, where the branch asks for one | Rs 100 non-judicial stamp paper and the notary charge. Two originals, because the office keeps one. Do not buy it before somebody asks for it | The same day |
| Fixing Aadhaar, PAN or the bank first, where those are the records that are wrong | Aadhaar charges a fee per attempt with no refund and limits how often a name may be updated. PAN and the bank have their own small charges | Set by each office |
| Board or school correction, where a certificate is the evidence and it is itself wrong | The board or institution own charge, plus Rs 10 for an RTI application where a register page has to be obtained that way. We never print the JAC correction fee, because the figures circulating online belong to other boards | Set by the board |
| Government printing charge, only in a genuine change of name | The guidelines print Rs 1,100 for an adult change of name and Rs 1,400 for a public notice, for FY 2016 to 2017, each with the caveat that the charge varies each financial year and from case to case. What BharatKosh shows on the payment screen the day you pay is the figure that counts | Part IV publishes weekly, so allow several weeks from posting |
| Our charge | Quoted on your papers before you pay anything, and confirmed in writing. We print no service price on any page of this site | |
The honest summary is that an LIC correction is rarely about money at all. It is about doing it while the life assured is alive, and about producing the original bond to a branch that can endorse it. The expensive version is the one that is never done, and is discovered by a family at a claim. Our name change cost page sets out the line items in full.
Send the policy bond and we will read both names
Photograph the policy bond page carrying the name of the life assured and the name of the nominee, and the page showing the policy number and the servicing branch. Then send the identity documents for both people, yours and the nominee's: Aadhaar, PAN and a bank passbook page. If there is more than one policy in the house, send them all, because the error is rarely on only one. Our first reply tells you which of the two names actually differs, what kind of difference it is, whether it needs a correction on the bond or a change of nomination on Form 3750, and the exact wording to put in the written request to the servicing branch. Details on the contact page.

Myths About LIC Policy Name Correction
| What people are told | What is actually so |
|---|---|
| There is an LIC name change form to fill in | There is not. LIC's published download list carries forms for claims, maturity, alterations, revival, assignments, loans and nomination, and no name change form. A correction is a written request to the servicing branch, endorsed on the bond if accepted |
| Only the policyholder's name matters | There are two names on the bond. LIC's own guidance tells policyholders to "Ensure that the nominees name is correctly incorporated in the policy bond", and the nominee is the person who has to prove their identity at a claim, without you |
| You can sort it out at any LIC branch | LIC says to contact the servicing branch of your policy for any servicing matter. Not the nearest branch and not a call centre. It is printed on your documents |
| The family can fix it after the death, with an affidavit | This is the belief that costs the most. The only person who could state with authority that both names are the same person is the life assured. Afterwards the family is proving an identity rather than correcting a record. Do it while the life assured is alive |
| A gazette notification is needed | No. A policy is corrected by endorsement, not by publication. A gazette matters only where you are genuinely changing your name across all your records, and even then the policy simply follows the change |
| The document list is standard, so any website's list will do | LIC publishes no procedure for a name correction, so practice varies between branches. A confident list on a website is one branch's habit written up as a rule. Ask your servicing branch in writing and let the answer be the list |
| Correcting the nominee's name and changing the nominee are the same thing | They are not. Correcting a spelling is one conversation. Changing who the nominee is is a separate transaction with its own numbered form, 3750. Keep them apart so neither is refused for being the other |
| Correct the policy to match the oldest certificate | Correct it to the form you are settling on everywhere, including Aadhaar, PAN and the bank. Matching an old certificate while the rest of your file says something else just moves the mismatch along |
Key Terms in an LIC Name Correction
| Term | What it means in practice |
|---|---|
| Policy bond | The physical policy document. It carries both names, and a correction is endorsed onto it, which is why the original has to be produced |
| Life assured | The person whose life is covered. Usually, but not always, the same person as the proposer who pays |
| Nominee | The person named to receive the money. The second name on the bond, and the one almost nobody re-reads |
| Nomination | The act of naming that person. LIC publishes numbered forms for it, including 3264 to nominate and 3265 where the nominee is a minor |
| Form 3750 | The notice of change of nomination. Changing who the nominee is, as against correcting how their name is spelt |
| Appointee | The adult appointed to receive on behalf of a minor nominee. Has its own forms, 3738, 3741 and 3740 |
| Endorsement | A change written onto the policy itself. How a name correction is recorded, because a policy is a contract rather than a database row |
| Alteration | LIC's own word for a change to policy terms, applied for in writing to the servicing branch |
| Servicing branch | The branch that holds and services your policy, and the only one LIC directs you to for a servicing matter. Not necessarily the nearest one |
| Death claim | The claim made after the life assured dies, where the bond, the death certificate and the nominee's identity are all compared against each other |
| Correction | Putting an existing entry right against documents already held. Publishes nothing, and what almost every LIC name case actually is |
| Received stamp | The branch acknowledgement on your own copy of the request. Without it, a request never happened |
